Blepharoplasty
is one of the most popular plastic surgeries, and this is not surprising. After all, everyone needs eyelid surgery: sooner or later, everyone experiences age-related changes in the eye area. Some patients have problems even at a young age due to the anatomical features of the facial structure.
Eyelid surgery is an operation that requires an individual approach based on a preliminary diagnosis of the facial structure.
However, first of all, it is necessary to understand what the main goal of patients going for blepharoplasty is. Of course, the main thing that patients, and plastic surgeons, strive for in their work is beauty.
Even in the distant past, the ancient Greek poetess Sappho said: “What is beautiful is good!”
Leonardo da Vinci saw beauty in symmetry, and Sir Francis Bacon asserted that “beauty is in harmony.”
The Oxford Dictionary defines beauty as “something distinguished by grace of form, charm of color; qualities that please the eye and evoke admiration in human faces and other objects.”
Of course, the concept of beauty among different peoples differs and changes depending on time.
I collected a number of the most famous paintings by Russian portrait painters, analyzed them and identified some features in all the women from the “canvases”. They have voluminous lower eyelids, and there is practically no line of transition from the eyelid to the cheek. Volumetric cheekbones. A clear contour of the lower jaw, a pronounced chin.
Having studied this, we radically changed our approach to facial rejuvenation and eyelids in particular. After all, what did eyelid surgery consist of before? In removing fatty protrusions, so-called “hernias” of the eyelids, excision of excess skin, and, practically, everything. Of course, heavy upper eyelids and lower eyelids with hernias and wrinkles began to look different. But have we achieved the rejuvenation that is natural and so desired? In most cases, no.
Moreover, what was considered a satisfactory result of blepharoplasty just a few years ago may today be a reason for correction as a consequence of an incorrectly performed operation. As a rule, a common result after standard blepharoplasty was the effect of the so-called round eye
or the effect of sunken eyes.

This effect occurs due to a downward displacement of the lower eyelid and an increase in the height of the palpebral fissure, which, as a rule, occurs due to excessive excision of skin and hernias of the lower eyelids. In this case, a white strip of sclera appears between the lower eyelid and the cornea of the eye, and, as is known, normally the lower eyelid should cover the lower edge of the cornea by 1-2 mm.
So, let’s study some of the results of an operation 10-15 years ago and try to analyze them.
It is certainly necessary to remove excess skin in the upper eyelid area, as well as the medial (internal) hernia. But, having removed the hernia of the lower eyelids, we did not shorten the length of the lower eyelid, did not eliminate the tear trough, the transition of the eyelid-cheekbone, eyelid-cheek was preserved.
And here is an example of a modern approach: pronounced changes in the area of the upper eyelids, the lower eyelids were distinguished by large hernias, excess skin and masking bags. And, of course, standard eyelid surgery would not give the desired result. Therefore, we performed a complex of rejuvenating operations in the midface area (lower eyelids – fat-preserving blepharoplasty with the elimination of the tear trough, cheek plastic surgery with the elimination of paint bags and shortening the length of the lower eyelid, myopexy to strengthen the lower eyelid, canthopexy for the same purpose and changes in the shape of the eyes) .
The patient had the same changes, which were less pronounced, but the approach was less radical (the patient did not have enough time for rehabilitation). However, almost all problems have been resolved. The patient refused the proposed canthopexy and myopexy; she did not want to change the position of the lower eyelid. Upper eyelid surgery and fat-sparing lower blepharoplasty were performed.
So why did we analyze previous patients in such detail? Can we conclude that standard blepharoplasty with excision of fat and skin has become obsolete? We think so! At least, the indications for its implementation are narrowed as much as possible. It is extremely rare that standard blepharoplasty is necessary, but, of course, such patients do exist.
For example, patients with minor excess skin, mild hernias of the lower eyelids. When performing standard blepharoplasty, we achieved a good aesthetic result, but it should be noted that the resection of fatty hernias was carried out extremely conservatively.
Today our approach to blepharoplasty is as follows. We do not consider the eyelids as a separate area on the face, but only in the complex of the middle zone of the face. We believe that a rejuvenating effect can only be achieved by preserving the “lower eyelid – cheek – cheekbone” complex, in which there should be no visible boundaries or transitions. And the youth of this zone directly depends on its volume and fullness.

Here is the most striking example.
A 25-year-old patient with lower eyelid hernias. A number of clinics recommended standard blepharoplasty for her. We offered her and carried out lipofilling of the tear trough, cheeks and cheekbones along the border with the eyelids, since we considered the main problem not the protrusion of hernias, but ptosis and thinning of the midface. That is, our approach in this case, as in many others, was not to delete and excise, but to add and fill.
So, instead of standard blepharoplasty, we use lipofilling as an independent technique in young patients with slight ptosis of the tissues of the midface, the presence of “hernial” protrusions of the lower eyelids. We also perform lipofilling for patients with previous blepharoplasty, a visible eyelid-cheek junction, and a visible lower orbital margin. For patients with the same manifestations of age-related changes in the eyelids, but with the presence of excess skin and weakness of the lower eyelid, we supplemented lipofilling with excision of the skin of the lower eyelids. If necessary, we supplement the operation with lateral canthopexy.
It is lipofilling that can highlight the result of any operation.
And here is an example of plastic surgery of the upper and lower eyelids with the movement of “fat bags”, due to which we perfectly rejuvenated the patient.
In conclusion, I just want to say that a patient, turning to a plastic surgeon, hardly wants to solve one problem at the expense of another: remove hernias of the lower eyelids and reveal the tear trough, or remove hernias of the upper eyelids and get (if not immediately, then in a year and a half guaranteed) sunken eye effect. Youth, naturalness and beauty are the patient’s main requirements for a plastic surgeon and, I am sure, modern approaches and methods can satisfy this request.
