Candidate of Medical Sciences Solomon Maisovich Abrahamyan has been working as a leading plastic surgeon at Frau Klinik since 2016. Corr.: Solomon Maisovich, you have been successfully operating for more than 10 years, more than 10 years. How did you come into the profession, why exactly maxillofacial surgery and specialization - rhinoplasty?
Solomon Abrahamyan: I chose the profession of plastic surgeon in my second year at the medical academy and never thought about another specialty. I became interested in facial anatomy, and realized that the most “interesting” operations for me would be nose surgery, facelift and blepharoplasty.
Corr.: In 2016, you developed a new method of nose surgery - microrhinoplasty. What are the indications for this operation? And how does it differ from traditional methods?
Solomon Abrahamyan: The indication for this operation is standard - dissatisfaction with the shape of the nose. Microrhinoplasty solves all aesthetic problems. Unlike classic rhinoplasty, this method is a more gentle surgical intervention. Tissue damage occurs at a minimal level, blood supply is not impaired, and sensitivity is preserved. The rehabilitation period passes very quickly - 7-9 days. The main advantage of “microrino” is the natural appearance and preservation of the natural anatomy of the nose as a result of the operation.

Corr.: Do you operate exclusively using the closed method? How many nasal surgeries have you performed in your surgical practice?
Solomon Abrahamyan: Yes, I operate exclusively using the closed method, since I consider this method the most harmless for the patient. I didn't count them, but I think about 2000.
Corr.: Tell us about the most interesting/difficult case?
Solomon Abrahamyan: There were so many of them. I’d rather tell you about an interesting one. At the end of last year, an old friend of mine, a cosmetologist, came to me for a facelift. At one time I operated on her nose. She really asked for a “doll”, small nose. In general, I am against such interventions. But in this case, she simply persuaded me. Then, at 33, she came to me for a facelift, I sent her to “grow old” (smiles). And now, 5 years later, I took on this operation. A timely facelift is a preventive measure. It allows a woman to preserve her youth without allowing many involutionary changes. Naturally, such an operation should be carried out only when indicated.
Corr.: How many patients do you have for whom you have performed more than 1 operation?
Solomon Abrahamyan: Quite a lot. Rarely, when a woman begins to take care of her appearance, she stops at one operation and procedure, especially middle-aged women. I have a patient who had a circumferential facelift, blepharoplasty, rhinoplasty, breast replacement and looks great.
Corr.: On the Internet they write about you that you often perform revision rhinoplasty, as well as nose correction after various injuries, can you tell us more about this? Do men often contact you?
Solomon Abrahamyan: People often apply for revision rhinoplasty. The most complex operations are precisely the “alterations”. Men apply less often. About 20% of all my patients are men. Mostly they want to get rid of hernias under the eyes or correct the shape of the nose. In our country, people are beginning to understand aesthetic and anti-aging surgeries. But not men. After all, male plastic surgery is still rare.
Corr.: Is there a difference in “female” and “male” rhinoplasty?
Solomon Abrahamyan: Specifically in my work, that is, in the process of the operation itself, there is no difference. But in the process of rehabilitation - there is. For men, the rehabilitation process is longer and more difficult. If a woman, after an extensive complex operation, for example, breast lift and abdominoplasty, already a few days after the operation begins to go about her daily activities, picks up a child, etc., then men, as practice shows, even after such gentle and careful operations , like microrhinoplasty, begin to feel better only after a few weeks.
Corr.: How to act in case of a nose injury? Is it possible to contact you about this issue? Or is this the task of a traumatologist?
Solomon Abrahamyan: Of course, you can see a traumatologist. He is a specialist who provides first aid. You can come to me - only immediately, on this or the next day, before extensive swelling sets in and the injury is “fresh”. In this case, the patient can be operated on almost immediately and restore the aesthetics and functionality of the nose. If enough time has passed since the injury for swelling to form, you will have to wait about 4-6 months to undergo rhinoplasty.
Corr.: Now you operate only with the microrhinoplasty technique? Or are there cases that require more extensive intervention?
Solomon Abrahamyan: Microrhinoplasty is a unique technique that combines all the advantages of already established working methods (closed rhinoplasty, endoscopically assisted rhinoplasty, etc.). Access through the nasal passages, video support in the operating room, quick anesthesia due to the high speed of the operation, and a short recovery period - these are the main advantages of nasal microsurgery. Previously, performing such an operation took surgeons about 1.5-2.5 hours. Now we have reached such a level that the time has been reduced to 50-90 minutes. And even in the most difficult cases, microrhinoplasty gives amazing results.
Corr.: How do you feel about non-surgical nose correction with fillers or absorbable drugs?
Solomon Abrahamyan: I believe that non-surgical rhinoplasty does not solve global aesthetic problems, much less functional ones. It is needed by those who want to get rid of minor shortcomings for a while, which sometimes interfere with life. So I am not against this method of nose correction (namely, fillers based on hyaluronic acid, and not absorbable drugs that deform the nasal tissue). Let's not forget about people who are afraid of surgery, the sight of blood, anesthesia, or have health contraindications for surgery.
Corr.: What criteria are used to evaluate the result of rhinoplasty? And what should a patient do if he is not satisfied with the result of the operation?
Solomon Abrahamyan: In plastic surgery, as in art, there are no standards. And the result is assessed according to two parameters - technique and aesthetics. Technique is the quality of the surgeon’s work, aesthetics is how the work looks. It happens that the patient does not like the high-quality work of a surgeon only because the perception is purely aesthetic, and in his dreams the patient did not see his nose like this. And it happens the other way around. That is why one of the main stages of preparation for surgery is a consultation with a surgeon; usually, I consult my patients for about 40-60 minutes - this is the time to understand as accurately as possible what final result the patient wants to see.
Non-surgical rhinoplasty does not solve global aesthetic problems, much less functional ones
If you are not satisfied with the result, you need to contact your operating surgeon and explain what exactly you don’t like. And the surgeon, in turn, must explain to the patient why “this” looks the way it does. It happens that the patient evaluates the result very early when the swelling persists, perhaps the presence of a scar process, which is very individual, or perhaps the rehabilitation process went wrong. Patient dissatisfaction is not always associated with poor performance by the surgeon. Since there are situations in which 2 operations are required to achieve the desired result, for example, with the same scarring or with thick skin in the nose area.
Corr.: What can you advise patients on choosing a doctor for rhinoplasty?
Solomon Abrahamyan: The main thing is that the patient of the rhinoplasty surgeon understands that the nose is the most expressive part of the face, and any changes in its size and shape lead to a change in the perception of the face as a whole. Any operation must be carried out according to indications, including rhinoplasty. Yes, sometimes we have to refuse patients, since there is no need to interfere with the natural anatomy of the nose, since rhinoplasty, in my opinion, is a noble operation that, on the one hand, leads to an ideal, and, on the other hand, quite radically changes the patient’s appearance.
