The education system for plastic surgeons in Russia is undergoing significant changes, and the new generation of young specialists will have to study this area of ​​medicine somewhat differently. In the meantime, domestic students are gaining knowledge, experts who received their education abroad are returning and working in Moscow. One of these international-level specialists is Boris Georgievich Mirzoyan. The doctor completed full specialization in plastic surgery in France and Switzerland in accordance with European training standards. Over his long career, the surgeon performed over 4,000 plastic surgeries. Why did the doctor return to Russia? What distinguishes the domestic and French education systems for plastic surgeons and are the needs of patients in Moscow and Paris different?

Corr.: Can you say that plastic surgery is your calling, and not just a profession?

Boris Mirzoyan: Working as a plastic surgeon gives me great pleasure. I am interested, and I am sincerely happy to do my job: managing patients, operating, observing post-operative rehabilitation, learning new technologies. I purposefully walked towards my profession. In fact, I already had a qualification as a urologist and was offered a job in France in urology, and subsequently in general surgery, but I refused, and I had to further specialize in university clinics for more than 6 years to get into plastic surgery.

Corr.: How did you want to choose this particular direction in medicine?

Boris Mirzoyan: In fact, the decision to become a plastic surgeon was made almost instantly. It was in a public clinic in Lyon. I walked into the plastic surgery operating room where a leading Lyon plastic surgeon was performing breast reconstruction. The patient had no breasts, and before my eyes, a plastic surgeon created breasts from the patient’s own tissue. It was a creative process of recreating a lost organ and with an artistic approach. It became clear to me that this was the profession I wanted to pursue, and a long journey of 13 years began to achieve the final goal.

Corr.: Tell us about how you got to France?

Boris Mirzoyan: I received an international scholarship. I had the opportunity to travel to any country in the world, and I chose France. I initially went for 6 months and in no case planned to stay longer. After 6 months, I realized that it was impossible to train in surgery during this time, and I had the goal of specializing in plastic surgery. The high quality of education in surgery kept me in France.

Corr. : Can we say that your development as a plastic surgeon took place there?

Boris Mirzoyan: Yes, definitely, I went through all stages of training in plastic surgery from the very beginning according to the French educational program. In accordance with the European education system, I supplemented my specialization with related specialties. In Switzerland I spent 2 years in general surgery and 6 months in otolaryngology.

Corr.: What difficulties may arise for a foreign surgeon operating in France?

Boris Mirzoyan : Difficulties for a foreign surgeon in obtaining a specialist certificate and administrative permission to work in France. In fact, there are no paid residency programs in any specialty in France. Places are strictly limited as they are paid for by the state. The number of residents in the departments is no more than 3-4 people, which extremely complicates the process of admission to the specialization. There is a strict competitive selection system. To hire a foreigner, according to the law, the employer must prove that he did not find a French citizen for this job. France is an administratively very complex country compared to other European countries.

Corr.: In which clinic did you operate in France? What is the working day like for a plastic surgeon in France? What operations did you perform? What patients came to you?

Boris Mirzoyan: The fact is that during specialization the resident is given the opportunity to choose and undergo an internship in university clinics every 6 months. This is a very correct system, in my opinion, so the resident has the opportunity to work in different departments and gain knowledge and experience of different techniques, and there are a lot of good clinics with outstanding surgeons. The working day is very busy; in public clinics work begins in the morning at 7.30 and ends late in the evening. Compulsory medical insurance covers a lot of plastic surgery operations, and therefore there are a lot of patients and a correspondingly large workload, but this is an essential condition for gaining experience. I have performed virtually all plastic and reconstructive surgery procedures.

Corr.: Where is it easier for a surgeon to work - in France or in Russia? Explain why.

Boris Mirzoyan: It’s difficult to answer this question, what do you mean by simpler? For any surgeon there is a period of formation, creating his own patient base. If a surgeon manages to get through the difficult initial period, which requires a lot of effort, quality work and time, then we can say that the job is easier in terms of job security, which is very important for maintaining technical skills. I think that in plastic surgery it is possible to organize well-organized work regardless of the country. The most important thing is to work in a team of professionals and trained personnel.

Corr.: What is the reason for your desire to move to Russia?

Boris Mirzoyan: In fact, I went to France to undergo training and specialization. I spent 13 years in France, and I wanted to return to my native countries.

Corr. : Russian realities in the field of plastic surgery are somewhat different from those in France. Was it difficult for you to adapt to them?

Boris Mirzoyan : At first it was a little difficult, for one reason - I was used to the French system. This concerned the names of drugs, materials and the work of nursing staff. In fact, it was more difficult for me, since I started working in a multidisciplinary clinic, where there was no plastic surgery at that time. A lot had to be organized. Subsequently, I visited and worked in Moscow in clinics exclusively for plastic surgery, where the work was very good.

Corr.: What can you say about competition in Russia? How did things go with this in France?

Boris Mirzoyan: Competition in Russia, of course, is very strong. This is due to two reasons. Firstly, there are a lot of plastic surgeons in Russia. According to some reports, several thousand; before 2015, it was possible to obtain a plastic surgeon certificate through professional retraining within 5-6 months. In France, the number of plastic surgeons is strictly limited; as of 2022, there were 953 throughout the country. The education system does not allow us to graduate more plastic surgeons. The process of doctors retiring and replacing them with new personnel has been established. Secondly, prices for plastic surgery can vary greatly in Moscow depending on the specialist and the clinic. In France, prices are approximately the same, there is no big variation, which also reduces the competitiveness factor.

Corr.: What operations do you carry out here most often?

Boris Mirzoyan : I have acquired multidisciplinary skills and successfully perform operations on the face, chest and abdomen. I consider the first priority to be the correct indication for surgery, patient safety and natural results.

Corr. : How do patients in Russia find out about you?

Boris Mirzoyan: Basically, word of mouth.

Corr.: Is the “Russian patient” different from the “French” one?

Boris Mirzoyan : The only difference that I would note concerns consultation. French patients come to a consultation with a clearly formulated request of what worries them and what they want to correct. In Russia, I am often asked the question “tell me, doctor, what I need to do,” of course, this applies more to facial surgery. I usually answer that no one needs anything regarding plastic surgery. Most plastic surgeries are aesthetic in nature, and they should only be performed if it really bothers the patient.

Corr.: How do you see your professional future in 5 years? And in 10?

Boris Mirzoyan: I would like to do university work, teach and train students.
