Corr.: Pavel Sergeevich, what facial plastic surgery is most in demand now?
Pavel Golovanev: There is no certainty in this matter, since the demand for the operation depends on each specific case. As a rule, people of a fairly young age come to me (from 35 to 45 years old) and quite often they have already done something to their face, for example, put threads.
And if they do not have severe ptosis, then it makes sense to offer them endoscopic facelift methods, for example, the forehead, midface. Most often people want to avoid major surgery and major surgery, and I think that's right.
Also, do not forget about recreating volumes using implants - for example, in the area of the cheekbones, lower jaw, etc.
Corr.: Have there now been any new techniques or even new operations to correct various facial imperfections?
Pavel Golovanev: As for new methods, here, most likely, we should talk about the materials with which this or that operation is carried out. After all, in principle, the mechanisms of the operation are approximately the same.
I recommend combining conventional facial plastic surgery with volume restoration in the cheekbone area
But it should be understood that any operation consists of many components, and the result will depend on how the plastic surgeon combines them. The doctor plans methods and schemes of intervention during a consultation with a certain patient - after all, for some, only an endoscopic forehead lift will help, for others, SMAS lifting will be necessary, and for others, simply Botox injections will be enough.
Corr.: Do you recommend combining any facial plastic surgeries? What does this give?
Pavel Golovanev: Yes. I recommend combining regular facial plastic surgery with restoration of volume in the cheekbones - this will make the lifting of the midface more pronounced. It is better to do this together with an endoscopic facelift.
In general, endoscopic techniques are perfectly combined with facial lipomodelling, since the volume of fat tissue is well redistributed, the necessary areas are filled, wrinkles and nasolacrimal grooves disappear.
Also, facial plastic surgery manipulations can be combined with Botox injections, various laser technologies and thread techniques - together this will give a good aesthetic result.
Corr.: At what age is facial plastic surgery most effective?
Pavel Golovanev: A lot depends on the patient’s genetics. At what age does tissue ptosis occur, what is the general predisposition to certain age-related changes. Lifestyle, ecology, stress also matter - all this can lead to early tissue involution and early aging.
In general, we can say that the recommended age starts from 35 years and every year there will be more and more manifestations of factors requiring aesthetic correction.
Corr.: How long does the effect of the operation last? Is it possible to do it once and the effect will last a lifetime?
Pavel Golovanev: Honestly speaking, I have never encountered cases where the effect of the operation lasted a lifetime. Although, if you get a facelift at the age of 80, it is very likely that the results will last for the rest of your life (laughs).
If you undergo an endoscopic facelift at the age of 35, you can count on 5-10 years of good results, of course, with proper self-care and a healthy lifestyle.
Corr.: To whom would you categorically not recommend undergoing facial plastic surgery?
Pavel Golovanev: I can say that if a person has not made a firm decision about plastic surgery, I would not recommend him to do anything at all, not just facial plastic surgery.
A person must understand for himself - based on his views and conclusions - whether he needs this operation or not, and then the surgeon will confirm or refute these thoughts. As for me, I think there must be indications for surgery.
If the patient does not have ptosis or any tissue involution, then surgery is not recommended. Also, if there are contraindications for health reasons. Since this operation is aesthetic, it is definitely not worth risking your health for it.
Corr.: Which facial plastic surgery is the most effective?
Pavel Golovanev: This should be considered individually, depending on the patient’s problems. There is no concept of the most effective operation - in one case it will be one, in another another, and in the third the most effective solution will be a combination of several operations and procedures. You can’t make a face according to a template, and you shouldn’t strive for universality either. It is important to maintain individuality, everything should look natural.

Corr.: Is there some kind of “gold standard” - a set of facial surgeries that can be recommended to almost everyone at a certain age?
Pavel Golovanev: There is no gold standard. There is a list of operations that a surgeon masters - each specialist has his own. And the surgeon’s skill lies precisely in being able to select a set of operations that will help most effectively correct the patient’s problem.
But at the same time, it is important that the intervention is unnoticeable, as if the patient simply became younger and more beautiful.
Corr.: Please tell us about the features of rehabilitation after facial plastic surgery.
Pavel Golovanev: The main feature is strict adherence to all the recommendations of the plastic surgeon - both before and after the operation. It is clear discipline that allows you to achieve the effective result that the doctor was striving for.
You should also be calm about such issues as swelling and hematomas - this is a normal phenomenon after surgery and over time it will disappear without a trace.
Corr.: Sharp cheekbones and thin noses are in fashion now, what do you think will be in fashion in the future?
Pavel Golovanev : Healthy and proportionate faces are always in fashion, so I think this will remain “in fashion”.
