As we enter our second year of wearing masks due to the global pandemic, plastic surgeons are receiving more and more patient inquiries about bags under their eyes and what to do about them. It seems that as our eyes have become the focal point of our masked faces, many people have started to notice that their under-eye bags have become more visible. With that in mind, we spoke with a plastic surgeon in St. Petersburg and asked Dr. Stanislav Ekimov about the "seamless" style of under-eye bag removal, as well as whether there are non-surgical options on the market.

Corr.: “Removing bags under the eyes without scars.” This expression is often found on the websites of plastic surgery clinics. Can you tell us what it is?

Stanislav Ekimov : When we say “no scars,” patients do not want a cut on their face that is visible to the naked eye. To do this, we approach the correction using a transconjunctival route, which runs from the inside of the lower eyelid. So we actually carefully flip the eyelid down and make an incision to access three pockets of fat. Depending on where the fatty hernias are located, we can choose how much we remove to create the desired aesthetic appearance.

Corr.: What equipment is used during this operation?

Stanislav Ekimov: There are many options that we can add to the removal of bags under the eyes. Essentially, for people whose tear trough area is severely sunken, we sometimes perform fat transfer to fill the void. In other cases, fat grafting may be used, where fat obtained from another area of ​​the patient is used to correct the infraorbital area and reinjected around the tear trough area. This gives a smooth transition visually and provides the added benefit of reducing the appearance of dark circles around the eyes.

Reporter: Can you tell us what causes bags under the eyes?

Stanislav Ekimov: Bags under the eyes are essentially genetic. I have seen some patients who develop this feature as early as adolescence, and others who develop it in their 30s to 40s and beyond, but the genetic component is the main cause. Aging plays a role in the sagging and loss of elasticity of the skin as bulges of fatty tissue begin to herniate, press on the skin and become noticeable. But if you have a genetic predisposition to developing fatty hernias, they will form faster, and aging will make them more prominent.

Corr.: Tell me, is it possible to talk about “different degrees of severity” of bags under the eyes?

Stanislav Ekimov: Yes, that’s why we noticed that patients come at different times and at different points in their lives when the bags under their eyes begin to bother them.

Corr.: How long does the operation usually take?

Stanislav Ekimov: As a rule, I need no more than an hour for transconjunctival blepharoplasty.

Corr. : And is this done under local or general anesthesia?

Stanislav Ekimov: 80% of patients do it under local anesthesia, it is very well tolerated. For those who are very anxious, we provide light calming therapy. Your upper eyelids are actually closed during the procedure, so it's not like you're sitting there watching me work on you.

Remember that one of the effects of local anesthesia is that it may cause slight double vision for the first few hours. So we have patients who stay in the clinic to let it go, and in some cases they have an eye patch applied to balance their vision. This goes away within 4 hours after the procedure.

Corr.: What is the level of pain?

Stanislav Ekimov: I would say that it is about 2/10 - you will feel pressure on the eyeball, but it is not pain, I would say it is mild discomfort.

Corr.: What about the recovery time?

Stanislav Ekimov : The duration of rehabilitation varies, but most patients who have undergone transconjunctival removal of bags under the eyes will return to work within 3-4 days. As for the bruise, it intensifies on the second day, but by the 3-4th day the bruise acquires a light yellowish color and is easy to hide with makeup.

Corr.: What risks are associated with this type of procedure?

Stanislav Ekimov: There may be swelling and bruising. Some patients who take a lot of vitamins and supplements actually bruise more. For patients taking anticoagulants, we ask that they stop taking these medications at least 5 days a week before surgery. We also advise patients not to exercise for the first couple of days and try not to cough or sneeze. Each of these actions increases pressure on the blood vessels, which can increase the bruise. When we see patients a week after surgery, they are doing well, healing progress is excellent, they can return to moderate exercise, but strenuous physical activity, such as working out at the gym, can only be resumed after two weeks.

Corr.: What is the average age of your patients who want to get rid of bags under their eyes?

Stanislav Ekimov: Honestly speaking, I had patients in their 20s. It is not uncommon for people aged 40 to 50 to come in, since some of them at this age still have excellent skin elasticity, and we do not need to do a classic lower blepharoplasty with an incision! But most patients who experience sagging and loss of skin tone will be over 50 years old, and the “seamless” technique will not suit them.

Dr. Ekimov’s patient underwent transconjunctival blepharoplasty of the upper and lower eyelids. One month has passed since the operation.⠀

Corr.: What about people who are not ready for surgery. Are there non-surgical methods for reducing eye bags?

Stanislav Ekimov : There is a method using radio frequency (RF) to eliminate excess fat. But often a course of several procedures is required to achieve an effect. I have had patients who tried a non-invasive method and then still came back for surgery because the results were not optimal. For this reason, I believe that if there are indications and possibilities, it is better to get by with blepharoplasty, which gives an expressive effect once and for all.

Corr.: Earlier you mentioned that you can do lipofilling for people with sunken eyes. Is it possible to inject fillers under the eyes of patients who do not undergo fat grafting?

Stanislav Ekimov: Fat transfer is a procedure to increase volume, just like fillers. The difference, of course, is that filler injections need to be repeated every 9 months to a year because they dissolve. One benefit of fat grafting is that it can reduce eye discoloration in some patients, while fillers can sometimes cause what is called the Tyndall effect, which causes a slight bluish tint in that lower part of the eye. Of course, the result of lipofilling lasts much longer than after contouring. But the final choice always remains with the patient.
