Flaws

Through the fault of the “doctor” Vares M.A. I went through seven circles of hell. I never recommend this doctor to anyone and, naturally, the clinic on Moskovskaya 19, where she works. My story began with the fact that an ultrasound in another clinic found a single polyp in the uterus (ultrasound was done several times - all times the ultrasound showed a single polyp without a doubt). Naturally, I intended to get rid of it, I was looking for a clinic where this could be done less traumatic for health and I found it on my own... During the call from Vares, which surprised me, she herself advised me over the phone. My choice fell on this clinic, since they offered removal of the polyp (versascopy) on an outpatient basis, under local anesthesia, which suited me quite well, the money was about 26 thousand. I came for “versascopy” and what do you think, when Vares started performing the procedure, suddenly " I found not one polyp in the uterus, but multiple ones! (There was only one on the ultrasound!) and said that in addition to multiple polyps, I have a real nightmare there, an overgrown endometrium, etc. etc., and, attention! Big fibroid! From where, excuse me? Grew up in a month?! And in order for me to remove it, I will need another operation under general anesthesia and for an additional 40 thousand, she wrote out the bill and calculation for me directly from the office where this was seen! Naturally, I was shocked by what I heard, but doubts about the reliability of the information naturally arose. As she told me, she supposedly removed the polyps for me, all that remained was to have surgery to remove the fibroids. As a result, I go for an ultrasound scan several times after the “versascopy”, each time the doctor says about a single polyp. Okay, I’m writing a review on the website of Moskovskaya 19. The administration of the gynecology department contacts me, they invite me to a conversation, I come with a complaint, I try to prove that I’m right, of course in vain. I leave the complaint to the lawyer. Naturally, this is constant stress, the problem has not been solved. I gather my strength, go to a trusted, reputable clinic for a hysteroscopy with one of the best gynecologists in the city and oh, miracle, there is no trace of fibroids!!! A single polyp, as it was originally, is so. The operation protocol, histology and other information after hysteroscopy showed a single polyp - excuse me, did the fibroid melt or something!? Vares didn’t even deign to remove it for me, while charging me a payment of 26 thousand, I’m not talking about the fears that she told me about the condition of my uterus and non-existent fibroids. All the data after a normal operation is documented, now I’m thinking about what to do next. In response to my claim, the clinic on Moskovskaya 19 refused to compensate for moral damages and the cost of “versascopy”. Then I did not have direct evidence, but now I have the results of hysteroscopy. I do not recommend this doctor or clinic to anyone! This is a money scam in order to milk the client, I don’t think this is the initiative of this doctor alone...

No, I’ll write anyway.:D Make yourself comfortable, gentlemen. From the latest: well, okay, Krokhaleva is young and with the least experience, but it’s unpleasant to run into a crooked and blind cosmetologist for the second time only with a quarter of her life’s experience in such an illiterate clinic; it’s no longer offensive, but outrageous. Everything could be attributed to different skin characteristics and other crap, but I’ve been riding the wave of cleansing for more than a year and no one has carried out such a mediocre technique. Like who? Like Srokina A.E. At the appointment, I immediately asked if there would be scars (what do you mean “will there be scars”?! A competent specialist will not allow them!) in several unpleasant places and received the answer no. In response, but not in fact. In fact, the cleaning was carried out poorly and poorly! Those same “unpleasant moments” crushed SO hard - literally to the bone. My pain threshold is adequate, I’m not afraid. But at that moment I immediately realized that I could not leave with such injuries without scars... The wounds bled both after cryomassage and at home. On top of everything else, a lot of pores remained clogged, including the obvious areas around those “moments”, because of which, collectively, I decided, reluctantly, not to wait for Boyarskaya to return from vacation and made an appointment with Sorokina. They wrapped me up to my chin in sheets and laid out irons. Of course, how can you see everything when you are completely covered. I have something to compare with. Boyarskaya V.S. master of cleaning. Checks every pore: on the contour of the lips, around the lips, around the ears, on the neck, etc. It doesn't hurt. There is no blood flowing out of the office. By the way, about the office. Unheard of impudence! Shouldn't you notify the client about the presence of a stranger at the reception? That is, instead of building a dialogue and warming up to the cosmetologist, I’m tense because of the intern and can’t discuss all my questions! Besides, it’s not the most pleasant thing in life to lie with your pores squeezing out under the soul of a stranger who is watching and taking notes. Are you crazy?! Offer a discount if you allow this or give the appointment for free. Give me back my money for that mediocre and useless treatment, which brought with it only troubles in the form of scars and new inflammations (even at the injection sites)! Pay 2550 rub. in such a clinic and not receive proper cosmetic care... how is this possible?! The hands of your cosmetologist are crooked and greedy! I thought, since she gave me scars, I’ll come to her to fix them? Look for sheep in another wallet. From the old (for dessert): my adventures in this clinic began when I was 16 with a dermatologist and later it became necessary to see a gynecologist. Rykova S. Yu conducted my first appointment amazingly, she sorted out a lot of things and opened my eyes, she was very kind and attentive. But the review is not about her, but about Yu. A. Berezovskaya. Do you know why you suddenly signed up with her? Because I needed a gynecologist-endocrinologist based on Boyarskaya’s recommendations. Afraid of making a mistake, several times during the appointment they clarified whether she was a gynecologist-ENDOCRINOLOGIST and answered “yes”. Arriving at the reception, I asked again - they approved and sent me to the office. When I got up, I found a sign saying “obstetrician-gynecologist” and asked my question to the woman in line, who shrugged. I had to go back down to the reception to clarify. I was reassured. As I was getting back up, a nurse circled me from behind, reassuring me that the appointment would be coming soon. Indeed, they let me in before that woman in line. The doctor immediately apologized for the wait. So that's it! The entire staff just said that I was outraged because of the queue (and my appointment time was really far gone, despite one more visitor ahead). What did I get? What was not required! I needed a specific recommendation on the skin and all the accompanying tests. In response to my question about what to do with my skin, they told me to go to a cosmetologist. Surprisingly, I came from her!:D Berezovskaya conducted an examination, which I did not need (since Rykova prescribed other days of the cycle for this!), did not explain anything about the available tests, did not talk and did not solve the problem even in gynecology. I prescribed a bunch of examinations and drops of Vigantol. I did not take advantage of all these recommendations and solved the problem in another clinic in one visit - banal endometriosis. I would like to get up and leave right away or not even go into the office, but you yourself know exactly how gynecologists love to take off their pants at the door and put them on the couch, and I was waiting with all my heart for them to give me recommendations on the skin... that she left in tears, having shelled out an extra 3,500 rubles. By the way, do you know what I later learned from Boyarskaya, who was indignant that I still had not visited the necessary specialists? It’s not surprising that you don’t have a gynecologist-endocrinologist in your clinic, because even on the website there’s not a word about it. Ok, even if she somewhere has competence in endocrinology, for some reason she turned out to be completely uninvolved in my problem. I gave money just to spread my legs one more time and even after examination I didn’t get any results. The “specialist” threw up her hands and said that she didn’t know why I was having cramping menstrual pain. Question two: Are you crazy?! What kind of case do your administrators send to the wrong doctors? By the way, they really are not all friendly and welcoming. For example, not long ago two people attacked me because I came with the groom’s card and didn’t know that I didn’t have the right to pay instead. They laughed at me and looked at me like I was a dependent beggar. Great job! I also demand my money for that long-standing useless reception from Berezovskaya! This, of course, is strange now after a while... but your specialists themselves are to blame. One situation is superimposed on another and the result is this kind of angry aftertaste. Let administrators bear financial responsibility for their mistakes, not the client. PS: For some reason, a gastroenterologist also does not require tests to determine the diagnosis and the overall picture.

Neurology

Appointment with a neurologist. Kinesio taping. Treatment using the “Dry Needling” method. FDM therapy. Botulinum therapy in neurology. Bruxism. Synkinesia, facial asymmetry. Migraine treatment. At the reception is neurologist Olga Sergeevna Shterner.

A neurologist is a specialist who specializes in the diagnosis, prevention and treatment of diseases of the nervous system.

You should definitely contact a neurologist if you experience:

  • headaches (migraine, tension headache and others);
  • facial pain, facial asymmetry;
  • dizziness, loss of coordination;
  • noise in the head and ears;
  • pain in the neck, back, upper and lower extremities;
  • weakness, decreased strength in the limbs;
  • sensory disorders;
  • visual impairment (double vision, loss of visual field, strabismus, etc.);
  • disturbance of smell, hearing, taste;
  • speech disorder;
  • anxiety and depression;
  • sleep disorders;
  • disturbances of memory, attention, thought processes;
  • syncope (episodes of loss of consciousness);
  • seizures;
  • tremor (shaking) of the limbs.

At the appointment, the neurologist collects complaints, anamnesis (information about the course of the disease and general health), assesses the neurological status - checks reflexes, muscle strength, sensitivity, determines the condition of the vestibular apparatus, etc. For this, special instruments are used and simple tests are performed. Everything is painless and does not cause discomfort. Based on the results of the examination, the neurologist may refer the patient for additional examinations: ultrasound of the neck vessels, radiography, MRI, CT, electroneuromyography, laboratory tests and other necessary diagnostic methods.

Treatment is always individual, depending on the disease, its stage, age and general health of the patient.

Botulinum therapy.

  • Botulinum therapy for chronic migraine
  • Botulinum therapy for neuralgia, trigeminal neuropathy
  • Botulinum therapy for postherpetic neuralgia

Kinesio taping.

Kinesio taping technique

founded by Japanese doctor Kenzo Kase in 1979. In 1988, at the Olympic Games in Seoul, Japanese athletes appeared with kinesio tape applications, after which the technique began to spread throughout the world. In 1995 and 1998, the method was introduced into the list of medical services in the USA and Europe.

Kinesio tapes are adhesive tapes made of 100% cotton and coated with a hypoallergenic adhesive layer. Activation of the glue occurs when heated to human body temperature. The thickness and elasticity of kinesio tapes are similar to the surface layer of the skin. The peculiarity of kinesio tapes is that they can stretch only in one direction - along the longitudinal axis.

Mechanism of action of kinesio tapes

Kinesio tape mechanically lifts the skin and subcutaneous fat at the site of application, thereby activating microcirculation in the connective tissue and intercellular space, accelerating the removal of metabolic products (including inflammatory mediators) and improving lymphatic drainage.

Kinesio tape affects numerous skin receptors, resulting in a reduction in pain.

Applications of kinesio tape can regulate muscle tone: completely relax the damaged, spasmodic muscle, or stimulate it.

Activation of skin receptors with kinesio tape promotes the activation of proprioceptors in muscles and joints, thereby improving their function.

Kinesio taping is used both in complex treatment and independently.

Main Applications:

  • Neurology: condition after acute cerebrovascular accidents, osteochondrosis of various localizations, curvature of the spine, mono and polyneuropathy of various origins, neuritis of the facial and trigeminal nerve in the acute period and in recovery, etc.
  • Traumatology and orthopedics: deformations and dysfunctions of the musculoskeletal system, consequences of injuries, etc.
  • Obstetrics and gynecology: pain relief from algomenorrhea.
  • Aesthetic medicine: correction of keloid scars, elimination of edema of vascular origin and lymphostasis.
  • Sports medicine: injuries, disorders of the musculoskeletal system, etc.
  • Medical rehabilitation, pediatrics: prevention of disorders and treatment of the musculoskeletal system, cerebral palsy, etc.

Contraindications to kinesio taping:

    Absolute:
  1. Malignant oncological process.
  2. Individual intolerance, skin reaction to the material, adhesive base of the tape.
  3. Open wounds, abrasions, various skin diseases at the site of intended application.
  4. Areas of phlebothrombosis.
    Relative:
  5. Diabetes mellitus, severe course.
  6. Severe renal, heart failure.
  7. Respiratory diseases.
  8. Pregnancy (provocation of labor when applied to the lumbar and abdominal area).

Dry needling treatment

“Dry needling” is an effective method for treating muscle pain without the use of drugs.

The method was discovered at the end of the last century. The founders of the method are the American surgeon David G. Simons, who was involved in research in the field of aviation and space medicine, and pulmonologist, cardiologist, and general practitioner Janet Travell.

When inserting a needle for the purpose of anesthesia, it was observed that a positive effect occurs before the injection of the anesthetic substance. The researchers tried to forego drug therapy and simply insert a needle into the painful area. It turned out that it works.

When a needle is inserted directly into a painful point, the so-called trigger, the muscle fiber is overexcited, followed by relaxation, and as a result, pain sensations decrease.

The Dry Needling technique is indicated for muscle strain. This condition can occur in any person due to everyday things: habitual incorrect posture, non-optimal motor pattern, etc.

At first, this method used standard injection needles. Soon, conventional needles were replaced with acupuncture needles, which hardly injure soft tissues, but the effect of trigger relaxation is the same.

Acupuncture using the “Dry Needling” method is not related to classical acupuncture. The method is based on neurophysiology, while acupuncture works according to the canons of traditional Chinese medicine.

The technique is minimally invasive and in most cases is well tolerated. There may be hematomas at the site of needle insertion, muscle soreness that goes away after a few hours.

There are, of course, contraindications. Among them are acute infections; disorders of the blood coagulation system; skin damage; pregnancy; mental illness.

FDM therapy.

Pain is the most common problem with which a patient consults a doctor. Pain in the back, leg, arm, neck, headache... One of the reasons for the appearance of pain is a violation of the fascia.

Fascia is a thin connective tissue membrane that covers all organs, blood vessels, nerves, muscles and joints. Violation of the fascia is caused by various external forces - stretching, compression, overstrain and others. FDM therapy is aimed at restoring normal function and structure of the fascia and, as a result, getting rid of pain and sensory disorders.

FDM therapy was developed in 1991 by American doctor and osteopath Stephen Typaldos. The basis of the method is understanding the “language of pain”, i.e. the patient’s own perception: where and how it hurts, what causes or intensifies the pain, what movement or position relieves the pain.

A specialist, an FDM therapist, performs treatment with his hands and, in some cases, with the help of special devices.

FDM therapy is a fast and effective method of treating the musculoskeletal system. Doctors work using the FDM method in many countries around the world: the USA, Canada, Europe, Africa. The “language of pain” is the same for people all over the world, you just need to be able to understand it. FDM therapy is also used with great success in professional sports.

Indications:

  • joint and muscle pain, including back pain and headaches;
  • limited mobility in joints;
  • sensitivity disorder.

Contraindications:

  • oncological diseases;
  • violation of the integrity of the skin at the site of the planned manipulation;
  • early postoperative period;
  • bone fractures in the acute period.

When performing some techniques, pain may occur, but they are usually weaker than those with which the patient applied. Sometimes hematomas may occur at the site of manipulation.

Botulinum therapy in neurology.

Botulinum therapy

- a direction of modern medicine that uses botulinum neuroprotein to treat various diseases and syndromes. Most botulinum therapists are neurologists and cosmetologists.

For what neurological diseases is botulinum therapy performed:

Myofascial pain syndrome of the face

– accounts for up to 35% of all facial pain. It manifests itself as one-sided constant pain of aching, squeezing, bursting nature. The pain can be localized in the parotid-masticatory region, temporal, frontal, post-auricular region, radiating to the orbit, oral cavity, teeth, neck, tongue, pharynx, shoulder.

Bruxism

- gnashing of teeth. The consequence is uneven abrasion of the hard tissue of the teeth, overload of the periodontium, muscle-pain syndrome of the temporomandibular joint, and dystrophic changes in the muscles of the masticatory apparatus.

Botulinum therapy as a treatment for bruxism not only allows you to get rid of teeth grinding and pain, but also improves aesthetic aspects:

  • square “heavy” face, shaved;
  • wide cheekbones;
  • fights reduction in the height of the lower third of the face, improves aesthetic proportions;
  • eliminates the “temporal pseudosulcus”;
  • reduces deep forehead wrinkles and wrinkles between the eyebrows.

Facial nerve neuropathy and its consequences:

facial asymmetry, synkinesis (pathological movements of the facial muscles of the affected side), contractures of the facial muscles.

With the help of botulinum therapy, it is possible to reduce or completely eliminate pathological contractions of facial muscles, improve facial symmetry, normalize hypertonicity of facial muscles, stop lacrimation, and thereby improve the patient’s appearance.

Chronic migraine

– a disease in which migraine attacks occur more than 8 times a month for 3 months or more; or the patient suffers from headaches more than 15 days a month for 3 months or more.

Botulinum therapy can reduce the frequency and intensity of headache attacks. The patient notices a positive effect from the treatment of headaches after the first procedure.

Hemifacial spasm

(short-term involuntary contractions of the muscles of one half of the face);
blepharospasm
(involuntary contraction of the orbicularis oculi muscle, which causes the eyelids to close);
oromandibular dystonia
(involuntary violent movements of the muscles of the oral and mandibular group).

Neuralgia, trigeminal neuropathy.

Postherpetic neuralgia.

Good tolerability of botulinum therapy, safety, absence of systemic side effects and long duration of action (from 3 to 6 months or more) are characteristics in favor of choosing this treatment method.

Bruxism.

Involuntary grinding of teeth during sleep, caused by spasm of the masticatory muscles, is called bruxism
.

The reasons for the development of bruxism in adults have not yet been precisely established; in medicine there are several theories - psychological, dental, gastroenterological and others. It has been noted that bruxism develops more often in those experiencing high psycho-emotional stress and stress.

How is bruxism diagnosed?

A person suffering from involuntary grinding of teeth during sleep does not know about it, since sleep is not interrupted. Pain in the jaws, teeth, chewing muscles, and headaches may indicate a problem. During a dental examination, a specialist can detect chips on the teeth.

Over time, in the absence of effective measures, tooth enamel wears away, increased sensitivity of the enamel, cracks, and wedge-shaped defects appear. Due to the increased load, the service life of fillings and orthopedic structures (prostheses) is shortened.

If you have bruxism, when planning dental prosthetics or installing veneers, it is necessary, first of all, to solve this problem so as not to damage the structure.

A neurologist can offer drug treatment for bruxism: microelements and vitamins that have a beneficial effect on the nervous system, as well as sedatives. In neurology, drugs that reduce motor hyperactivity of muscles have been successfully used for a long time.

Treatment of bruxism with Botox (botulinum toxin).

To relieve muscle spasms, botulinum toxin is used: targeted injections of the drug block the nerve impulses responsible for their occurrence. It should be understood that the effect of the drugs is not permanent, up to about six months, so the injections are repeated if necessary.

Synkinesia, facial asymmetry

Damage to the facial nerve, referred to as facial neuropathy, occurs due to various reasons: Bell's palsy, facial neuritis, trauma, penetrating procedures, consequences of operations, as well as bacterial and viral infections, metabolic disorders, hormonal imbalances.

The main manifestation of facial nerve neuropathy is facial asymmetry caused by unilateral weakness of the facial muscles.

In the acute stage, the face is asymmetrical not only when moving, but also at rest. The healthy side pulls on the weakened muscles, causing additional discomfort.

In the later period, contractures of the facial muscles may occur, manifested in a feeling of tightness, tension and spontaneous twitching in the affected muscles, as well as synkinesis - pathological movements of the facial muscles.

How does synkinesis manifest itself?

Often they learn about this not only from a neurologist, but also from loved ones, from others who notice these manifestations. When talking or eating, squinting of the eyes occurs; when closing the eyes and blinking, the corner of the mouth rises, the nose wrinkles, and the subcutaneous muscle of the neck contracts.

What is the cause of synkinesis?

Neurology gives the following answer to this: in the pathological regeneration of facial nerve fibers, as a result of which several groups of facial muscles contract simultaneously.

An effective method for treating synkinesias is botulinum therapy, which has been widely used in neurology for more than 20 years.

How is the treatment carried out?

The procedure is carried out in 2 stages:

1. Botulinum neuroprotein preparations are administered in minimal doses to each zone of synkinesis. The most common injection areas are the upper and lower eyelids, the nasolabial fold, and the junction of the perioral muscles. Higher doses of botulinum neuroprotein are injected into the cords of the subcutaneous muscle of the neck. Contracture zones are also injected in minimal doses.

2. After 7-10 days, the doctor evaluates the effect, and, if necessary, corrects the remaining synkinesis and asymmetry. The effect lasts from 3 to 6 months or more. After the effect of the drug ends, the procedure is repeated.

The same method is also used to correct facial asymmetry.

It should be understood that there are no symmetrical faces in nature. We are all asymmetrical from birth, some to a lesser extent, some to a greater extent, and if there are no organic reasons, this is normal.

We are talking about asymmetry caused by facial neuropathy or paralysis. In this case, injections of botulinum neuroprotein are placed in the most active facial muscles on the unaffected side of the face.

Which drug should be preferred?

Our specialist (and this is a neurologist, it is this specialist who treats neurological pathologies) and his patients have a choice - Botox, Dysport, myotox. All these drugs have Russian registration and show high effectiveness. For the patient, the only thing that may be noticeable is the difference in the final cost of treatment. The price list can be found on the clinic’s website.

If any of the problems mentioned above bother you, come for a consultation with a neurologist.

Treatment of migraine with new drugs based on monoclonal antibodies

Drugs based on monoclonal antibodies are new drugs that appeared in the USA and Europe in 2018, and in Russia they go on sale in 2022.

How do monoclonal antibodies help with migraines?

Drug molecules act on specific “targets” in the body’s cells – a special CGRP protein or its receptor. It is this protein that is the main pain mediator of migraine and is responsible for the spread of pain during a migraine attack. New drugs block the pain pathway that involves the CGRP protein.

In our clinic we perform injections with the drug Erenumab (Irinex in Russia, Aimovig in the West).

This drug is recommended for the prevention of migraine in adults with at least 4 days of migraine attacks within 1 month.

The duration of the course of treatment for migraine is from 6 to 12 months, the drug is administered subcutaneously once a month. The effect is assessed 3 months after the start of treatment. Even in the case of a quick effect, it is recommended to take a full course.

What is the effect of the treatment?

Drugs based on monoclonal antibodies are preventive therapy. The goal of treatment is to reduce the frequency and intensity of headache attacks and reduce the number of painkillers taken. Unfortunately, it is currently impossible to completely get rid of migraines. But you can take control of the course of the disease.

In 20% of cases, migraine attacks completely end by the end of the year of treatment. There were 75% fewer seizures in approximately half of the patients participating in the studies, and 50% fewer seizures in 2/3 of the patients. Compared to other treatments currently available (antiepileptic drugs, beta blockers, candesartan, antidepressants and botulinum neuroprotein), these results are very good.

What are the side effects?

The most common side effect (noted by up to 20% of patients participating in the studies) is a reaction at the injection site in the form of redness, soreness, and burning. This side effect goes away quickly. Otherwise, the drug did not show any serious adverse effects.

The drug is recommended to be administered under medical supervision.
