Expert: a doctor’s reputation is made up of reviews from his colleagues and patients
How many Russian women need the help of a reconstructive surgeon?
Why did French queens hire wet nurses for their children? Does a healthy lifestyle affect the condition of the mammary glands? Doctor-mammologist of the Republican Clinical Oncology Dispensary of the Ministry of Health of the Republic of Tatarstan, head of the department of reconstructive plastic surgery of the Onkoprofi medical clinic, Doctor of Medical Sciences Artur Ismagilov, spoke about this in an interview with a RIA Novosti correspondent. — Artur Khalitovich, how many breast surgeries are performed at the dispensary annually?
– About 500 – 550. For Russia these are significant figures; we are considered one of the leaders in this field. Our specialists began performing such operations back in 1998. If we talk about the structure of patients’ requests to us, then almost every second one needs the help of a reconstructive surgeon. As a rule, reconstruction is carried out using expanders, implants, and much less often - flaps.
Among the remaining operations, corrective ones predominate: in 70 percent of cases we are talking about breast augmentation, and in 30 percent about its reduction and shape correction. For the most part, our client’s age does not exceed 45 years.

Manturova: imposing beauty standards leads to “barbization” of peopleChief freelance specialist - plastic surgeon of the Ministry of Health of the Russian Federation, head of the department of plastic and reconstructive surgery, cosmetology of cellular technologies of the Russian National Research Medical University named after. N.I. Pirogova Natalya Manturova shared her impressions of the major International Congress of Plastic Surgeons with RIA Novosti correspondent Yulia Osipova.
– How many women sign up for aesthetic correction after childbirth?
- Yes, a lot. Women's breasts tend to sag after childbirth. While breastfeeding is an important factor that has a positive effect on breast health, it has a negative impact on the appearance of the mammary glands. It is no coincidence that French queens hired wet nurses for their children.
– Some experts insist that sports and a properly formulated diet can prevent illnesses in this part of the female figure. Do you agree?
- It's a delusion. By resorting to a special diet or doing fitness, you can influence the muscular corset and body weight. But the mammary gland is a hormonally dependent organ; it primarily depends on hormonal changes in the body, and not on the amount of physical activity.
– Why is it believed that during pregnancy a woman’s breasts are invulnerable to deformation?
– During this period and at first during the “segment” of lactation (feeding), the ovaries do not produce female hormones that “bomb” the mammary gland, due to which the latter is at rest. Accordingly, the longer a woman’s pregnancy period lasts, the more intact her breasts are.
And yet, do not rush to classify female hormones as unfavorable factors that negatively affect health. After all, they are what make a woman a woman.

Choosing a plastic surgery clinic: what you need to pay attention to How to choose a plastic surgery clinic, the ria.ru infographic, prepared by the Social Navigator of MIA Rossiya Segodnya, together with the Institute of Plastic Surgery and Cosmetology, will tell you.
– How widespread, according to your observations, is the fear of plastic surgery among the fair sex?
– What are you talking about, he remained forever in the 1990s. Today there is a boom in plastic surgery. A visit to a cosmetologist or plastic surgeon is becoming good form and an indispensable component of a modern woman’s schedule. Moreover, not necessarily a business woman. This could be a teacher, an engineer, or a lawyer.
– How long does the operation usually take?
– From one and a half to three and a half hours. But it is not considered heavy in the usual sense; its peculiarity is different. Healthy people who carefully monitor their appearance lie on the table. Therefore, the price of this service is ultimately determined not by technical, but by psychological factors. By and large, we carry out psychological rehabilitation using a surgical method.
Let me return to your question. The operation is not done for speed, but to get the breast that suits first the surgeon and then the patient. This is why one “aesthetic” patient is more difficult for me than ten oncological ones.
– In what cases is there a need for reconstructive surgery?
17 October 2016, 12:28
Reshetov: we are “building a bridge” from life before oncology to life after. A plastic surgeon told how reconstructive plastic surgery can help cancer patients.
– For congenital underdevelopment of the mammary gland, its deformation after an accident, natural disasters. Or when this organ was removed as a result of treatment.
For a woman, this is associated with pronounced psychological trauma, which is not surprising. The bust was and remains one of the symbols of her femininity and sexuality. Therefore, in twenty percent of patients who decide to undergo surgery for oncology, the fear of losing the mammary gland prevails over the experience of their diagnosis.
– The percentage of cancer detection in Tatarstan is generally higher than in Russia. What do you attribute this to?
– Tatarstan is one of the few regions where the Republican screening program operates. It is aimed at examining healthy women. The goal is to detect preclinical or early forms of breast cancer. Unfortunately, it does not cover one hundred percent of the region’s residents, but it significantly increases the number of cases of detection of early forms of the terrible disease.
In general, screening is a very expensive and technologically complex undertaking. Not every European country can afford it, but Tatarstan could. The experience of foreign countries shows that where 80 percent of women participate in screening programs, mortality is reduced by a third. Clinical examination, organized in the region at a very decent level, also makes its contribution to improving early diagnosis.

The best plastic surgery clinics in Moscow - 2016The navigator presents 147 organizations, of which 21 are public and 126 are private.
– What is your position in connection with the appearance of the first independent rating of Moscow clinics this fall?
– Moscow is oversaturated with offers of services from plastic surgeons. This market is a little wild. Advertising aimed at patients is not always correct. To help them, of course, they need a rating based on the opinions of reputable professionals.
In the Social Navigator rating, the element of such assessment on the part of my respected colleagues is expressed more clearly, which is why I am inclined to consider it the most objective.
When preparing these materials, it is important to exclude advertising opportunities for the surgeon or the clinic he represents. So that patients can focus specifically on the professional capabilities of the service provider.
– Is an analogue of this study needed in other regions, including Tatarstan? The “Social Navigator” project of MIA “Russia Today” is preparing a similar rating for cities with a population of one million.
– In the regions the situation is simpler – there are fewer populated areas. If a surgeon works long enough, his name acquires a reputation that speaks for itself and is reflected on the Internet.
Let's say he does a hundred operations a year and 20 percent are not very successful. Reviews on the Internet are left by the most active patients. If they fall into a negative percentage, they can tarnish the good name of the surgeon. And vice versa, a doctor may have only 20 percent of high-quality operations, but if those operated on turned out to be active Internet users, then the picture with reviews will turn out to be impeccable.
In addition, in any case, the client should not be tormented by doubts about the reliability of the recommendations posted on the sites. What if, they say, the surgeon himself, through relatives or third parties, places a flattering description of himself? Therefore, for me, assessing the professional environment is decisive. If top-class surgeons, who have confirmed their reputation with years of experience, speak of someone with respect, then this is definitely a high-status surgeon.
– The market for “plastic” services is still quite green and wild, to use your definition. What serves as a navigator for the consumer?
– A significant number of my patients learn about the possibilities of plastic surgery not even from magazines and newspapers, but from social networks. As a result, many make an appointment with a cosmetologist for no apparent reason, because “a friend was there and she liked it.”
I can confirm, however: the new map of services on the market improves the intimate life of Russian women. This is one reason to visit a cosmetologist. But there is another one. The vast majority of patients come to us because they need to improve their self-esteem. They want to feel more comfortable and confident, and we are happy to help them with this.
