IPL facial photorejuvenation: pros and cons - appointment with a cosmetologist, contraindications to the procedure - MEDSI

Photorejuvenation is a hardware rejuvenation method based on intense pulsed irradiation of the skin.

This procedure has a number of advantages over other methods. Firstly, photorejuvenation is a non-invasive procedure, as a result of which there is no long rehabilitation period. Secondly, it is painless and safe. There were no reported cases of complications or side effects. Thirdly, skin rejuvenation using the Lumenis M22 device has a prolonged effect. Under the influence of light waves, the processes of synthesis of substances necessary to prolong youth are launched in skin cells.

Photorejuvenation: the essence of the procedure

Photorejuvenation is a way to get rid of facial skin imperfections using a special device that emits light pulses of a given frequency and length.
The procedure is carried out by directing various attachments to problem areas. Pulses in contact with skin cells activate processes such as:

  • synthesis of collagen and elastin;
  • oxygen absorption;
  • detoxification;
  • death of pathogenic microorganisms;
  • coagulation of blood vessels and their disappearance;
  • destruction of pigment.

The intensity of the impact is adjustable.

IPL Use Cases

Now there are such rejuvenation options, determined by what needs to be corrected:

  • reduction of pigmentation and treatment of vascular pathologies;
  • smoothing wrinkles, improving skin tone and texture, narrowing pores.

All this is achieved due to the fact that photons act on the lower cell plates deep in the epidermis. This effect improves the synthesis of elastin and collagen. At the same time, tissue nutrition improves.

During therapy, the deep layer of the epidermis is treated

Indications for light rejuvenation procedures

Exposure to intense light pulses, otherwise known as IPL therapy, is effective for:

  • the presence of age-related changes in the skin, which usually occur after 30 years (wrinkles, decreased firmness, elasticity, appearance of a double chin);
  • problems with young skin that cannot be solved by any other method;
  • unhealthy complexion;
  • dry skin, peeling, tightness;
  • hyperpigmentation (spots, freckles, melasma, etc.);
  • vascular network;
  • acne due to the ineffectiveness of drug treatment or as part of complex therapy;
  • increased sebum production;
  • enlarged pores.

Indications for use are quite extensive. However, only a doctor can predict the result in each specific case after examination and communication with the patient.

List of contraindications

The desire for beauty is absolutely natural for a person, especially a woman. However, in the pursuit of perfection, you can do harm. To avoid health problems and irrational spending, contraindications to facial photorejuvenation should be taken into account:

  • very dark skin by nature;
  • recent tanning (no more than 2-3 weeks ago), use of self-tanning;
  • acute dermatological and infectious diseases;
  • herpetic rashes;
  • oncopathology;
  • blood diseases;
  • presence of a pacemaker;
  • pregnancy and lactation period.

The procedure is also not recommended in summer, when there is intense ultraviolet radiation. Combination with phototherapy can cause the opposite effect - hyperpigmentation.

Pros and cons of the procedure

The main advantages of IPL rejuvenation methods are the following facts:

  • low traumatic procedures;
  • high effectiveness in the fight against skin imperfections;
  • the ability to prevent the occurrence of defects, wrinkles, etc.;
  • absence of pronounced discomfort during the procedure;
  • do not leave scars after light exposure;
  • do not require lifestyle changes or cessation of work;
  • long lasting effect.

Photorejuvenation is an excellent cosmetic method to solve many aesthetic and medical skin problems. However, it is not without its drawbacks. Among them are:

  • an impressive list of contraindications;
  • unpleasant, albeit temporary, consequences of the procedure in the form of swelling, redness, itching in areas bordering the mucous membranes;
  • inability to use cosmetics in the first 3-5 days after light exposure;
  • risk of photodermatosis, manifestations of hypersensitivity.

When does IPL fail?

It makes no sense to use this technique in the following cases:

  • sagging skin;
  • deformation of the oval of the face due to subcutaneous fat;
  • insufficient body fat and poor muscle tone;
  • pronounced deep wrinkles in the nasolabial triangle.

For the greatest effectiveness of the technique, the doctor may recommend additional use of others. For example, contour plastic surgery or biorevitalization.

It is worth combining photorejuvenation and another cosmetic procedure

Preparatory activities

To prevent the development of complications after the procedure, doctors at our clinic recommend stopping tanning in any way 14-30 days in advance. You should protect your skin from exposure to UV rays, even if you are not going to the beach or solarium specifically for this. Cosmetics can help you.

You also need to stop taking for a month:

  • non-steroidal anti-inflammatory drugs with antiplatelet properties;
  • antibiotics;
  • retinoids.

Nothing complicated. There are no other restrictions.

conclusions

Unfortunately, many cosmetologists and clinics, acquiring their own websites and other Internet resources, are increasingly misleading potential patients about certain procedures. There is no such thing as “laser photorejuvenation”, and you must remember this. This term is equivalent to mixing photoepilation and laser hair removal. And if you are offered “laser photorejuvenation” instead of laser rejuvenation, know: either this clinic does not have varied equipment and is deliberately confusing you with definitions, or, even worse, the cosmetologist himself does not understand the topic.

Striving for a positive result and health safety, choose a good cosmetologist and a clinic with licensed equipment.

Progress of the photorejuvenation procedure

All manipulations will take no more than 15-40 minutes. To begin with, the skin is cleansed with special non-aggressive products, then a gel is applied to it to facilitate the penetration of light rays and protect the epidermis and other tissues from overheating. To protect the cornea, the patient and doctor wear glasses. The doctor begins to work with attachments equipped with sensors. In this case, the patient does not experience severe pain, but the session cannot be called pleasant. There is a feeling of tingling, slight burning. To speed up regeneration and relieve redness, apply a cream with dexpanthenol.

Where to get fractional laser rejuvenation in Moscow

Contact the City Beauty and Health Center “Veronika Herba”, which uses innovative technology - the AFFIRM laser device, developed by the American company CYNOSURE. This laser can be considered a revolution in aesthetic cosmetology.

This laser provides non-ablative fractional laser rejuvenation. The US Food and Drug Administration has approved this procedure. It is also registered in Russia. In addition, this system has received the prestigious ASLMS Award and is recognized as the best fractional technology in the United States.

Fractional laser AFFIRM is:

Uniqueness. Deep rejuvenation + effective lifting in one session.

Possibilities.

The laser acts on all factors that contribute to skin aging: dilated blood vessels, wrinkles, skin hydration, pigmentation, decreased tone, uneven texture.

Intelligence.

The laser has a built-in triple radiation calibration system, which ensures accurate operation.

Maximum efficiency and economy.

The amount of microthermal damage increases the density of the laser beam and reduces the time of laser facial skin rejuvenation to 15 minutes. Thus, in one session the master performs two procedures.

Safety.

Since the penetration depth of the beams can be precisely controlled, the results are guaranteed. A big advantage is that the procedure does not require a long rehabilitation period.

Painless.

Anesthesia is not required because an air system is provided that cools the area of skin that is treated with the laser by 30 C. Thanks to this, the patient does not feel pain.

How does it work and how is it different from other procedures?

Lasers that use patented MultiPlex (MPX) technology generate two types of pulses. This is what makes them different from all others. The American laser Affirm MPX (Cynosure) has a wavelength of 1320 nm and 1440 nm, which allows you to consistently, with a difference of 3–5 ms, act on different layers of the patient’s skin, and therefore on different links leading to aging.

The procedure requires a glass tip, which provides cooling of the epidermis when exposed to the laser and after treatment. Cynosure Affirm MPX first emits a pulse at a wavelength of 1320 nm. The CAP system consists of approximately a thousand diffractive elements and is a special lens. These elements help disperse the laser beam, so large areas of skin can be treated in one pulse. The pulse with a wavelength of 1320 nm is redistributed, which leads to the fact that the dermal structures located at a depth of 0.5 to 2 millimeters are heated evenly, and regeneration of collagen fibers occurs. The skin becomes more elastic.

When a pulse with a wavelength of 1320 nm is applied to the skin, a pause lasting 3-5 milliseconds occurs. During this procedure, the epidermis is cooled and the client’s discomfort is minimized. After this, the laser emits a pulse with a wavelength of 1440 nm, which is also redistributed by the diffraction elements of the ACS. Thanks to this, the surface layers of the dermis and epidermis are treated evenly, and photodamage is corrected.

As you can see, the combination of MultiPlex technology and the CAP system makes the laser rejuvenation procedure more effective, without damaging the tissue.

Rehabilitation period

Redness, swelling, and small bruises characteristic of the recovery period disappear on their own within 2-14 days. For most patients, rehabilitation takes 3-5 days. Some do not experience any unpleasant symptoms at all after the procedure. However, in any case, the skin is subjected to stress, and many processes associated with rejuvenation, cleansing, depigmentation, etc. begin to actively occur in it.

Therefore, doctors recommend:

  • drink more purified water, since when the skin heats up, the liquid evaporates, and the rejuvenation process is impossible without hydration;
  • give up/limit yourself from smoking (nicotine and other components of cigarette smoke and tar negatively affect the condition of blood vessels, complexion and neutralize the effect of the procedure);
  • For about two weeks, avoid visiting the bathhouse, sauna, beach, or solarium.

If your general health deteriorates due to photorejuvenation, persistent swelling, hematomas and other symptoms, you should consult a doctor.

How to remove rosacea? Experiment on the BBL device, “before” and “after” photos

The BBL device promises to quickly and painlessly (well, almost) remove blood vessels on the face. We checked whether this is actually true.

As part of the project “Removal of rosacea and rosacea,” three BI readers and editor Lena Korenkova completed a course of BBL photo procedures. They were worried about the same problem: blood vessels.

As Dr. Yulianna Kulturbaeva said, in the case of Raisa Altasheva (39 years old), this problem worsened due to physical activity in a stuffy room. Raya works as a cleaner in a dentistry and beauty salon. He constantly bends over, and this makes the blood rush to his face more.

Marina Kirillova (46 years old) has a face that burns from temperature changes and any physical activity. The same problem is familiar to her mother and grandmother.

In the case of Bela Kaloeva (35 years old), hormonal imbalance most likely played a role.

Editor Lena Korenkova (28 years old) is just beginning to visualize her capillaries. There are a couple on the wings of the nose, but when the skin is stretched they are also noticeable in the central part of the face. History of atopic dermatitis. After washing, the face turns red and the overall skin has a pink undertone.

Left side of the face - immediately after washing with soft milk. The redness subsides within 10-15 minutes.

After examining the patients, Dr. Yulianna Kulturbaeva came to the conclusion that the situation was more serious than rosacea. All patients have a chronic disease, rosacea. (What is the difference between the first and the second, we wrote in detail here.)

All patients underwent four BBL procedures, three weeks apart.

The device affects the skin using light waves. As a result, the vessels are heated to the temperature at which they coagulate. The peculiarity of BBL is that the device allows you to select a specific wavelength and aim precisely at the target. That is, the doctor can set a high power of light energy and achieve an effect without the risk of burning the skin.

How does the BBL procedure work?

It was the same for me and other volunteers.

First, Yulianna applied a cool gel to her face, and I thought it was somewhat reminiscent of preparing for an ultrasound. In principle, this was not far from the truth. The gel improves the conductivity of the light beam.

I didn’t see what happened next, because I had safety glasses on my eyes :) Fortunately, Yulianna commented on the process in detail. The task of N1 was to warm up the skin so that the capillaries expand.

To do this, the doctor installed a suitable filter attachment and walked the device over the entire face, section by section. It was even nice. The BBL crackled monotonously, I felt warm, and I was close to taking a nap.

She probably would have fallen asleep if Julianna had not switched the BBL to the vessel removal mode. It feels completely different here. Each flash is like a blow from a tiny boxing glove. And hot too.

But I wouldn't say it hurts. I didn’t feel any pain as such. Only sudden fear, as if beyond my control. Because of the powerful flash in the face, my hands and back were cold. A few seconds later everything happened again.

Although here, for example, I tried to smile, and it seemed to work :)

Julianna changed the nozzle to a small one. She could work on the area near the wings of the nose and other hard-to-reach problem areas where the highest light energy is required.

The sensations have become sharper, but some 6-7 flashes are nothing, you can be patient.

The procedure takes 10-15 minutes at most. In fact, it took me longer to write this text than to lie on the couch.

I had few capillaries on my face, so I left the clinic without redness or any signs of intervention at all. I applied cream with SPF and went on my way to run errands. But this was not the case for everyone.

If there is more obvious vascularity, the face will appear red and slightly swollen after the procedure.

Therefore, Raya, Marina and Bela received LED therapy immediately after BBL.

Its meaning is that you lie under such a light lamp for half an hour, and all processes in the skin - including rehabilitation and tissue healing - proceed faster.

A feature that everyone, including me, noticed: each subsequent BBL procedure is less sensitive, the skin reacts calmer each time.

Dr. Kulturbaeva: “For the first time, we remove the maximum number of both large and small vessels. When you come for the second procedure, there are fewer of them. Accordingly, rehabilitation is easier.”

“After the first procedure, almost all the vessels were gone. However, by day 2-3 I looked wow. The face was very swollen, the cheeks became plump and hard. At the pharmacy they asked: “What’s wrong with you?” And, for example, a neighbor immediately understood - a laser. She said: “Nothing, it happened to me too, it will pass soon.” According to the doctor, the swelling could have worsened due to the fact that I went to work immediately after the BBL. So I signed up for the next procedures on weekends.

But the end result is worth it. Previously, I almost didn’t wear makeup, I used a thick foundation, and that’s it. Now I bought eye shadow, pencils, mascara, and several lipsticks! I took a lighter tone and even a highlighter (I still didn’t know what it was). And I admit, I had an ambitious idea to change jobs. Not right away, of course. With time. But I'm thinking about it."

Let's compare BEFORE and AFTER:

Yulianna Kulturbaeva: “Temporary compactions in the affected area are normal: lymph has accumulated at the site of injury.

I usually recommend maintenance treatments every three months. However, Rai's work will continue to provoke the growth of the capillary network. In her case, it is better to visit them more often, once every two months.”

“Rehabilitation after the first procedure was the most unpleasant. On days 2-3 the bruises darkened, and on days 4-5 the swelling increased. I think it's better for those who work in an office to sign up for BBL on Wednesday evening. Then the peak of troubles will be on the weekend, and by Monday they will subside.

Pleasant bonuses that I did not expect: my pigmentation has lightened and my face has stopped swelling in the morning. Doubly satisfied!”

Let's compare BEFORE and AFTER:

Yulianna Kulturbaeva: “Vessels can react differently to the same impact. For one person they seem to close, for another they burst. Therefore, Raya, for example, did not have bruises after the procedure, but Marina did.

Rosacea is characterized by chronic tissue inflammation, so patients with this disease are prone to swelling. BBL eliminates the cause of inflammation, leaving the face looking slimmer and tighter.”

“Very pleased! I liked the effect after the first and even more so after the third procedure, but the fourth, final, effect was even better. It seems that I have found my cosmetologist. I am absolutely delighted with Julianna!”

Let's compare BEFORE and AFTER:

Yulianna Kulturbaeva: “The nose is the most difficult area for photo procedures. On the one hand, to remove the vessels, powerful light radiation is required. On the other hand, there is almost no subcutaneous fat on the nose, and there is nowhere for energy to dissipate. You need to work carefully so as not to cause burns. Considering all these difficulties, we achieved the best possible result.”

“The main news: I got rid of the annoying pink complexion. A wonderful, wonderful world without limits! They used to exist. Despite the fact that I loved (and love) very light tonal colors, my piggy essence always shone through them. Sometimes I layered it more densely. More often, I took something with a stronger camouflage or with a gray-earthy pigment (it neutralizes redness well). Less often, I scored and walked around as is (mostly when I had a tan:).

Finally, my face is not pink and I hope the effect lasts as long as possible. I try not to forget about SPF for this.”

Yulianna Kulturbaeva: “Lena should have taken preventive measures so that rosacea would not move to the next stage. If she returns to BBL at least once every four months, the growth of the vascular network will be avoided."

LazerJazz doctors offered help to another volunteer, our photographer Kristina Spivak, who has a rare vascular pathology - hemangioma. We will show the course results in the fall. Stay with us!

Skin care after the procedure

To rid yourself of possible consequences such as photodermatosis, swelling, inflammation, and to prevent their occurrence, you must:

  • in the first days, refrain from applying decorative cosmetics;
  • to cleanse the skin, use only tonics and milks that do not contain alcohol;
  • use medicinal and care gels, creams and ointments prescribed by a doctor;
  • Go outside only after applying a high-SPF protection product.

Your skin could use support from the inside too. It is useful to take vitamin and mineral complexes, the composition of which is aimed at activating the process of regeneration, rejuvenation, strengthening blood vessels, etc.

Optimal number of sessions

The composition and duration of the course depend on the type and severity of cosmetic defects and are determined by a cosmetologist in each case individually. To remove local changes (spider veins, age spots), 1-2 procedures are sufficient. When eliminating wrinkles, preventing the appearance of acne in the future, or correcting sagging skin, 3, 6 or even 10 visits to a specialist may be required.

There must be a certain interval between sessions. On average, it ranges from 2 to 4 weeks and is determined individually by a cosmetologist.

FAQ

How to relieve swelling after the procedure?

Swelling is a normal reaction of the body to exposure to light. The permeability of the cell wall of the dermal vessels increases, the fluid leaves its previous place of residence and migrates. We see this as an increase in the size of certain areas of the face.

In the vast majority of cases, this reaction goes away on its own after 3-4 days. To alleviate the condition, you can use cold compresses, antihistamines (ask your doctor which ones), and soothing creams/gels. Herbs (herbal infusions) that have a diuretic and anti-inflammatory effect (for example, lingonberry leaves, erva woolly grass) also help relieve swelling.

Is it safe to do SMAS lifting after photorejuvenation?

These hardware procedures are perfectly combined. Photorejuvenation cannot affect the beauty of the oval of the face, eliminate sagging in the cheekbones, remove a double chin, or get rid of jowls. But ultrasonic SMAS lifting is capable of this. However, it is important to maintain an interval between procedures of 14-30 days.

Is it possible to do photorejuvenation after Botox?

Yes, you can. Botulinum therapy is effective against facial wrinkles and can prevent their appearance. And phototherapy copes well with age-related changes such as hyperpigmentation, uneven complexion, and “reformats” the skin into youth and beauty. However, the hardware procedure should be carried out 2-3 weeks after the injection. During this time, Botox will have time to “stand up” and maximize its effect blocking the transmission of nerve impulses.

What is forbidden to do after photorejuvenation?

After the procedure, you should not sunbathe, take a steam bath, sauna, drink alcoholic beverages, use aggressive cleansers, or medications without the consent of your doctor. It is important to take care of your skin, which has been exposed to intense light.
