Preserve beauty, provide care: Irkutsk plastic surgery experts

Petr Viktorovich, how long ago did Clinic Expert Irkutsk have a plastic surgery department?

— It has been functioning since its opening. At first there were three plastic surgeons in the department, now there are five. We have a very strong team. Our clinic performs almost the entire range of popular plastic surgeries, including intimate plastic surgery, liposuction, lipofilling, plastic surgery of the abdomen, buttocks, mammary glands, ears, eyelids, face, neck, scar removal and more.

What specialists work here?

— The department is headed by Yulia Evgenievna Oleynik. She performs operations on the mammary glands (breast enlargement and reduction, correction of the nipple-areolar complex, etc.), abdominal wall plastic surgery, and contour plastic surgery. Andrey Vladimirovich Oleynik is a plastic surgeon and specialist in maxillofacial surgery. I have also been working in the department from the very beginning. My experience in general surgery is 20 years, in plastic surgery - 12 (since 2008).

Over the past year, two more specialists have joined us. Mikhail Vitalievich Rozhok is a mammologist, oncologist and plastic surgeon. He is one of the leading specialists performing major operations on the mammary gland, including reconstructive ones. Patients who have survived breast cancer can count on full, qualified care. Mikhail Mikhailovich Staritsyn also has extensive experience in general and plastic surgery.

Simultaneous operations can be performed at the Expert Clinic Irkutsk. What are they and what are their advantages?

— These are operations in which several doctors simultaneously operate on different organs, sometimes even on different parts of the body. Combinations vary depending on the patient's needs. So, we recently worked together with Yulia Evgenievna Oleynik, and quite often we perform operations together with gynecologists. Thanks to simultaneous operations, a person does not have to prepare for a new operation, having barely recovered from the previous one, or undergo anesthesia twice or more often.

Examination before plastic surgery can take a lot of effort and time. How are things going with this at the Expert Clinic Irkutsk?

— Our clinic employs doctors of various specialties, provides a wide range of diagnostic services (laboratory tests, functional diagnostics, medical imaging), and has modern high-quality equipment installed. In addition, there is good communication between doctors, including those from different departments. There is continuity: the patient is seen by one specialist, but other doctors are involved in the process, whose knowledge will help the patient at this stage. Therefore, preoperative examinations do not cause much trouble.

They say you have a luxury hospital. What does this mean in a medical setting?

— We have a good hospital: single rooms, each room has a bathroom with a shower, Wi-Fi works. But I don’t really like the “luxury” characteristic. I think these are just normal conditions. Every person needs care and comfort, and especially when recovering from surgery. This must be kept in mind when designing medical facilities. We do everything to ensure that patients not only receive qualified medical care, but also feel comfortable in the hospital. Caring for people is the norm of life.

How to choose breast implants?

One of the areas of the body that representatives of the fair half of humanity pay special attention to is the chest.

Petr Viktorovich Shelest, Deputy Chief Physician for Surgery at Clinic Expert Irkutsk, Chief Plastic Surgeon of the Irkutsk Region, Candidate of Medical Sciences, talks about the role that breast implants play in preserving and improving her beauty.

— Pyotr Viktorovich, in the 80s, first American and then European clinics faced a wave of lawsuits. They were submitted by women after breast correction operations because their breasts began to deflate five years after mammoplasty. The lawsuits involved liquid silicone implants. What breast implants are used now?

Modern types of breast implants belong to the 4th-5th generation according to various classifications. They are made of dense, cohesive medical grade silicone. It has the so-called “shape memory” and does not have fluidity. In addition, the shells of current products are practically impenetrable.

The situation you described can be explained not only by the use of liquid silicone implants. At the beginning of the use of such operations, so-called saline breast implants were widely used, i.e. filled with normal saline sodium chloride solution. Over time, they slowly but steadily lost volume due to the seepage of part of the solution through the implant shell.

— Tell us about the pros and cons of the breast implants you listed

The main disadvantage of any implant is that it is a foreign body for the body. All possible risks and consequences are associated with this.

The main advantage, compared to the same liquid implants, is the stability of the gel form. This allows you to get the desired and long-term result.

— Is the service life of breast implants limited?

Yes. At the same time, the world's leading manufacturers provide a lifetime guarantee for certain situations associated with their products - for example, spontaneous rupture of an implant (i.e. without apparent causes from the external environment). If this happens, they provide a new product for free.

You need to understand that over time, changes occur in the tissues and the implant itself. It is impossible to determine its exact service life. The average is about 10 years. However, this does not mean that upon reaching this period the implant must be changed: indications are needed for replacement.

The average lifespan of a breast implant is about 10 years.

— An implant rupture on an airplane due to pressure or while sleeping on the stomach is a subconscious fear of almost every woman who has recently undergone mammoplasty. Which breast implants rupture more often than others? Are there such statistics?

Modern implants on airplanes and when lying on your stomach under the weight of your body do not rupture. So you can use them even in space.

— Based on your experience, can you say that any type of implant is definitely better?

These are, first of all, implants approved by well-known medical communities and expert organizations. One of these most recognized organizations is the American FDA (Food and Drug Administration).

— The more expensive the implant, the better it is? Or does such a dependence not always exist?

Not always, but in a significant number of cases. In other words, more often than not, the higher the cost of breast implants, the better they are. These are products that have generally passed FDA inspection.

— How can a patient avoid making mistakes and choose the right breast implants?

First of all, contact an experienced surgeon who performs such plastic surgeries.

— Petr Viktorovich, in the matter of choosing breast implants, who has the last word: the patient or the doctor?

In practice, the choice largely depends on the doctor, although, of course, this issue is always discussed with the woman. It is necessary to accurately understand the goals and practical feasibility of a woman’s wishes.
