From time immemorial they say in Rus': “The eyes are the mirror of the soul.” And it’s not in vain that they say. It is enough to look carefully into the eyes of your interlocutor to understand whether this person is open-minded or on his own, has come with goodness or is harboring anger. And it happens that a person wants to lie, but cannot - his eyes give him away.
And it is our eyes that can treacherously reveal our true age and the burden of all our problems. The fact is that the skin around the eyes is thin and delicate, and the muscles and ligaments are weak, and sometimes even in young people you can notice small wrinkles, which are popularly called “crow’s feet.” As a rule, crow's feet are a sign characteristic of cheerful and open people who smile a lot. But they also don’t want to grow old ahead of time. Moreover, after some time, this “sign of cheerfulness” can lead to the fact that the outer corners of the eyes simply droop and the gaze takes on not a cheerful, but, on the contrary, a tired look, and the gaze becomes “heavy.” And no matter how well you take care of your skin, no matter how expensive creams you use, sometimes you cannot do without surgical intervention.
Let's figure out why the eyes become “sad” and how can this be corrected?
Plastic surgeons say that the tired appearance of the eyes is due to the fact that the outer canthus (corner of the eye) droops.
The tendons that support the outer canthus are long and thin, and over time they can weaken and stretch. As a result, the corners of the eyes droop. And it happens that these tendons have a very weak tone by nature, in which case the baby is already born with “tired” eyes.
To correct the situation, you can resort to plastic surgery called canthopexy , that is, tensioning the outer corner of the eyelid. This same procedure may be called canthoplasty. The difference between them is that during canthoplasty, the tendons of the outer canthus are not tightened, but the excess is partially removed and stitched back together.
Most often, canthopexy is performed during plastic surgery on the eyelids and is, in fact, an integral part of blepharoplasty.
Indications and contraindications
Canthopexy has its own indications and contraindications, just like any other operation.
Thus, canthopexy is indicated in the following cases :
- if the corners of the eyes droop with age or due to genetic predisposition;
- during blepharoplasty of the lower eyelids, when it is necessary not only to remove “bags” under the eyes, but also to tighten the lower eyelids, eliminating wrinkles and possible sagging;
- with excessive stretching and sagging of the lower eyelids;
- if the patient wishes, create a so-called “cat look”.
Canthoplasty also helps with some diseases.
In case of myopia, for example, after a canthus lift, the “bulging eye effect” disappears. It is also capable of removing the consequences of paralysis or an unsuccessful operation, which resulted in the so-called ectropion of the eyelid.
Under no circumstances should canthopexy be performed for conjunctivitis and other inflammatory eye diseases. The remaining contraindications are the same as for other elective operations:
- poor blood clotting;
- exacerbation of acute or chronic diseases;
- diabetes mellitus from the stage of decompensation.

- And what is the result?
A.Sh.:
During the operation, a higher position of the outer corner of the eye and a more toned appearance of the tissues of the lower eyelid are achieved. Rehabilitation is the same as for eyelid surgery. Bruising and swelling disappear within two weeks. The feeling of unnatural tension goes away after 2 months. By this time it will be possible to fully evaluate the result.
— After your description, a contradictory impression is created, because many plastic surgery clinics offer such a correction as simple and effective...
A.Sh.:
Yes, indeed there are now a lot of advertising statements from clinics that they perform canthopexy.
You always need to pay attention to two points: first, is it necessary to do this
(is there any objective evidence)?
and second, is this actually being done?
After all, what is the point - in contrast to the genetic structure or age-related changes in the orbit, ligamentous apparatus and the place of fixation of the outer corner of the eye, the surgeon needs to destroy all this and move it, as a rule, to a higher position. The operation is quite traumatic, very rarely actually indicated, because often, having removed the overhanging skin on the upper eyelids, eliminating hernias and drooping tissues of the cheekbones, the changed appearance is quite satisfactory for patients. On the other hand, if surgery is performed at the muscle or skin level, it will not be canthopexy. Perhaps this is a more serious version of blepharoplasty, but it is impossible to say that canthopexy was performed in this case.
Anvar Shukhratovich, thank you for the detailed explanation, I am sure that now our readers will understand the nuances of eyelid canthopexy.

Canthopexy techniques
Today there are many methods by which canthopexy is performed. But only a plastic surgeon has the right to choose one or another surgical technique.
Typically, the outer canthus is tightened through small incisions above the upper eyelid. The seam remains in the crease of the eyelid, so it is practically invisible. Typically, this procedure is performed during upper blepharoplasty, which allows you to perform 2 manipulations at a time - to lift the corner of the eye and rid the upper eyelid of excess skin, for example.
If we are talking about sagging lower eyelid, then the plastic surgeon can cut off excess tissue near the outer corner of the eye and sew up the incision, while tightening the eyelid itself. The seam is also practically invisible, since it is located in the corner of the eye.
Canthopexy, like blepharoplasty itself, is performed under local anesthesia and the patient goes home on the same day.
Canthoplasty (canthopexy) - correction of eye shape
Plastic surgery does not stand still, so every day people all over the world have a chance to become more perfect.
Recently, the procedure of canthopexy and canthoplasty has become very popular. Canthopexy and canthoplasty allow you to change the shape of the eyes, tighten the corner of the eyelids, thereby achieving an aesthetic appearance of the entire face. The procedure received its name due to the fact that any operation performed in one way or another affects the canthal tendon in the eyelid area, which is responsible for support.
This procedure is most popular among people who have severe ptosis (sagging eyelids) and too narrow/wide eye shape. Canthoplasty is also suitable for those who have drooping eyelids due to age-related changes. Canthopexy helps to “rejuvenate” the look, since the procedure tightens the lower eyelid and also removes ectropion (a case where the eyelid is slightly inverted).
Rehabilitation after surgery
Do not think that canthopexy is just a cosmetic procedure, after which no rehabilitation is needed. A rehabilitation period is required . You will have to spend several days at home - you cannot bend over, and under no circumstances lift anything heavy. In other words, it is necessary to adhere to all the requirements and rules that are indicated during the rehabilitation period after blepharoplasty.
And, of course, after canthopexy, postoperative bruises cannot be avoided, but they will all go away within 7-10 days. Contopexy can be performed at any time of the year. In this regard, the most suitable time for surgery is spring, summer and early autumn. You can safely walk around in dark glasses without feeling awkward.
A week with dark glasses, and now everyone can see that your look has changed and your eyes have begun to smile.
What is contopexy
Every woman wants her eyes to be alluring and captivate any man. In youth, it’s easy to maintain the beauty of your eyes: properly moisturize this delicate area, which is prone to the earliest appearance of signs of aging. But the skin around the eyes itself is thin and contains a small amount of collagen. Tonic, cream, mask - with the help of this complex the area around the eyes will be in normal balance until a certain point. With age, irreversible changes occur in the skin and soft tissues of the upper and lower eyelids, as a result of which blepharoplasty surgery becomes the only radical way to rejuvenate the area around the eyes. Today, there are many types of eyelid surgery, and now we will focus on one of them. We will talk about canthopexy.

Postoperative period
Canthopexy and canthoplasty are minimally invasive procedures and can be performed on an outpatient basis. In the postoperative period, the doctor prescribes antibiotics to the patient to prevent the development of infection, analgesics if pain occurs, and eye drops to prevent irritation of the conjunctiva.
During the first few days after surgery, patients are concerned about swelling and bruising in the intervention area. On the first day, it is recommended to apply an ice pack every hour for 5-10 minutes.
In addition, special regenerating and antiseptic creams and ointments are prescribed to speed up the healing of the wound.
Recommendations in the postoperative period
- Try not to be in open daylight for more than 5 minutes in summer and spring; it is recommended to wear glasses with sun lenses in the first months
- Limit, or better yet completely eliminate, rest in front of the TV or laptop screen for a while, and, again, temporarily minimize reading books
- Avoid decorative makeup and wearing corrective lenses for a period of time.
- Avoid going to the sauna or working out in the gym, so as not to provoke excessive sweating and inflammation of the postoperative wound.
The stitches usually heal quickly. The effect of the operation is noticeable already in the first or second week. Final recovery occurs after 2-3 months, when the scar is completely regenerated and becomes invisible. The result lasts for 10-15 years. A very respectable period for the low price offered. The muscular-ligamentous frame of the eyes is ultimately securely attached to the periosteum.
Operation procedure
The intervention lasts 1-2 hours. It is performed under general anesthesia or local anesthesia (rarely and only for individual indications). The surgeon marks the area of canthoplasty or canthopexy with a special marker. After this, an incision is made in a natural fold. Through this access, manipulations are carried out with the tendon, skin, and subcutaneous fat. At the end, cosmetic stitches are applied, the skin is treated with an antiseptic, and a dressing is done. After 5-7 days, the sutures are removed if self-absorbable suture material was not used. There are no visible scars left because access is made through a natural fold of skin.
The rehabilitation period is quite short and easy. Special drops are prescribed for dry eyes. Swelling and hematomas go away on their own. The pain syndrome is relieved with a drug prescribed by the doctor. Restrictions apply to watching TV and reading, as well as other activities that put strain on the eyes.
The achieved effect is assessed after a month, it lasts up to 10 years.
