First wrinkles around the eyes, what to prefer: Botox or hyaluronic acid? (Expert opinion)
There is an old rule in aesthetic medicine that without preliminary preparation of the skin, Botox can only be injected into the forehead and only in patients under 30 years of age. I really like it because it is based on knowledge of the basic physiology and anatomy of facial skin. The skin around the eyes is very thin (approximately 3 times thinner than the skin on the cheekbone), contains few sebaceous glands that provide the secretion of natural nourishing lubricant, and, in addition, the skin of the eyelids is subject to constant facial stress when laughing. When the first wrinkles appear around the eyes, you first need to restore the skin using a course of mesotherapy or biorevitalization, and then consolidate the achieved effect with a Botox injection. Mesotherapy is performed in a course of 4-6 procedures once every 10-14 days. For mesotherapy of the skin around the eyes, special cocktails have been developed, which we successfully use for our patients with the first wrinkles in a short course: MesoEye, RRS eyes, F-eye contour.
Biorevitalization of the area around the eyes is not carried out if there is a tendency to swelling, as well as with preparations with a high concentration of hyaluronic acid due to the danger of malar edema (in the area of the zygomatic bags). If I do botulinum toxin without a preliminary course of preparation for patients who have no time to go for course procedures, then I prescribe medicated eyelid serums with vitamin C in the morning and retinol at night (for example, from the Medik8 line) for home care. Hyaluronic acid in the form of filler is not injected at the first wrinkles around the eyes , because the surrounding skin is very thin and prone to fibrosis (the growth of scar tissue). A filler such as Redensity II can be injected into the nasolacrimal trough (if present) using a cannula. As practice shows, this also leads to the smoothing of wrinkles, especially on the lower eyelid. Features of the structure of the periorbital region
The skin of the periorbital area does not have subcutaneous fat and is intensively supplied with blood, which leads to increased metabolism of the area and intensification of all processes, including withering. Therefore, wrinkles appear around the eyes very quickly, even with a slight loss of turgor and elasticity.
The muscles in this area are divided into two categories: oculomotor and facial muscles. Considering the importance of fast and accurate eye movements and focusing, which occur constantly during wakefulness, the muscle load can be considered very high. In this case, the facial muscles are involved both in purely facial movements (surprise, smile, anger and many others) and in oculomotor movements (raising the eyebrow when looking up and the like).

And given that the skin here is very thin and does not have subcutaneous fat, expression wrinkles form on it early. In addition, the orbicularis oculi muscle is practically fused with the skin. With age, it acquires hypertonicity, which leads to “corrugation” of the skin above it, as well as to a rounding of the eye shape.
Fat fiber
As already mentioned, the skin in this area does not have its own subcutaneous fat, and therefore is not protected by this shock absorber from mechanical load. But under the skin there is periorbital fatty tissue. Like any other adipose tissue, it is prone to swelling. Bounded along the posterior and lateral surfaces by bone structures, it protrudes forward during swelling, so even slight swelling in the periorbital region immediately appears. If the fascia that holds the periorbital tissue weakens, then hernias form. During swelling, the periorbital fatty tissue compresses the lymphatic vessels, further aggravating the situation. Therefore, swelling in the periorbital region is characterized by severity and persistence. Regular swelling and protrusion lead to stretching of the skin in this area.
