Is breast augmentation surgery safe? How long will it take for the scar to disappear? Will the implants need to be replaced over time? Glamor asked these and many other questions to eminent doctors from leading Moscow clinics.
Breast augmentation is one of the most popular plastic surgeries in the world today. In the United States, over the past five years, the number of people who have had it done has increased by 40%; last year alone, about 364 thousand women decided to do it. In other words, almost a thousand operations were performed per day! These are the data of ASAPS - American Society of Aesthetic Plastic Surgery. In Russia there are no such statistics yet - our “plastic” market is still rapidly developing, clinics appear and disappear, many operations are not taken into account, and such a profession as a plastic surgeon does not officially exist at all. So in the “specialization” column on the certificate of even the best doctor it will be written “cosmetic surgery”. Experienced breast surgeons, as well as representatives of the largest manufacturers of breast implants, helped Glamor understand this new, but not yet streamlined area of medicine and obtain the most professional information for Glamor.
If you have firmly decided that you need to enlarge your breasts or change their shape, and you cannot do without a surgeon, this does not mean at all that you can go and have an operation tomorrow. You should visit many doctors first. And you need to start with a consultation with your future surgeon, who will tell you everything about the upcoming operation.
Valentina Zmazova, a plastic surgeon of the highest category, chief physician and deputy director of the Institute of Plastic Surgery and Cosmetology of the Ministry of Health of the Russian Federation, says: “We usually perform breast augmentation for those whose breasts are deformed after feeding, due to improper development, after undergoing operations or inflammatory processes . That is, we try not to operate on nulliparous girls unless necessary. In addition, we suggest that every woman weigh the pros and cons several times. Mammoplasty is a very serious surgical intervention, and no one knows one hundred percent how it can turn out.”
After the initial consultation, the potential patient is sent for a full clinical and laboratory examination: she is examined by a therapist, gynecologist, mammologist, oncologist, dentist and anesthesiologist, she undergoes various tests and has an ECG.
In addition, good clinics usually employ a psychologist, but not everywhere they are required to be referred to one. Tair Rafikovich Aliev, head of the Center for Plastic Surgery, Cosmetology and Weight Correction “Clinic of the Century,” explains: “We have a full-time psychologist, but we cannot force a woman to go to him, my consultation costs 500 rubles, and a psychologist’s 1000. Another thing is that there is a category of patients whom I simply will not accept for surgery without a certificate from a psychologist.”
Based on the results of an examination by any of the above doctors, you may be denied surgery. Patients with mastopathy need to undergo breast ultrasound and mammography. Patients with hypertension, diabetes mellitus or varicose veins need to undergo a more complete examination and consultation with an endocrinologist, cardiologist or phlebologist to decide on the possibility of plastic surgery. If the patient has a cold, fever, or menstruation, the operation will be postponed to another day.
The final stage is the signing of three contracts: for surgical intervention (a standard contract for any operation), for physiological intervention (anesthesia) and for medical intervention (so that there are no complaints against the doctors if suddenly complications arise during the operation and the patient has to be given an IV or some drugs).
The most interesting stage for the patient is choosing the shape and size of the future breast. Here you need to keep in mind that implants come in two types: with silicone or with saline solution. Silicone is thicker (and therefore the breasts will feel more natural to the touch), saline is safer, but at the same time it is liquid, like water, and can gurgle when moving. In the USA, until 2006, silicone was banned and only saline implants were used, but in Russia the entire practice of mammoplasty is based on silicone. As for the size of the implants, it does not correspond to bra sizes (1, 2, 3, etc.), but varies in volume and is measured in milliliters. But the patient does not have to understand all the intricacies - it is enough to bring the surgeon a photo of the “ideal” breast. Another thing is that it is unlikely to be possible to repeat the picture exactly: the doctor is not able to change the shape of your chest and nipples.
The next stage is breast modeling on a computer or using silicone pads placed in a bra. You have to choose the shape of the implants: round ones look more sexy and provocative, anatomical (teardrop-shaped) - more natural. Tahir Aliyev explains: “It is difficult to advise which form to prefer. Anatomical implants are one and a half to two times more expensive, but not definitely better. For example, I think that they are like a bullet with a shifted center of gravity. In a dream, a woman turns on her left side, then on her right side. And the risk that over time such an implant will become loose and move inside the breast is higher than with a round one. Which can lead to various complications.” The surface of the implant is very important - smooth or rough: a smooth one is more likely to start rotating than a rough one.
The last thing to choose is where to make the incision: under the breast, under the armpit, or around the areola. There is no practice in Russia to insert an implant through the navel - due to the complexity of such an operation and the love of Russian doctors for silicone: after all, only a saline implant can be inserted through the navel. It is rolled into a tube, inserted through the incision, and then filled with saline using a catheter. But whatever implant you choose, the most important thing is to get a certificate for it with a lifetime guarantee and all the data: article number, size, date of manufacture, manufacturer’s contacts.
The size of implants is measured in milliliters. The smallest are 90 ml, the largest are 740 ml. The most popular: 200 ml (increase breasts by 1.5 sizes), 300 ml and 400 ml.
The operation is performed under general anesthesia and lasts one and a half to two hours. First, sizers are inserted into the breast - empty shells that are filled with sterile saline solution to once again ensure that the size of the implant is correct. Then the actual implants are placed. Based on the size of the breast and the thickness of the muscle, the surgeon decides where to insert them. If there is little gland tissue, then go deeper, under the muscle, otherwise the edges of the implant will be felt. Tahir Aliyev explains: “If the thickness of the gland is a centimeter, and the implant is one and a half centimeters, then do you think it will be palpable? Of course it will be. If there is a lot of it, then the implant under the gland will be natural both in appearance and to the touch.”
After surgery, you should remain in bed for 24 hours. Better yet, stay in the hospital for two days. “In a hospital it is easier to track and prevent typical complications: hematoma, thrombophlebitis, suppuration,” says Valentina Zmazova. But Tahir Aliyev recommends staying in the hospital for two nights only if the implant was inserted under the muscle: “Antibiotics and painkillers may be required. After all, in 75% of women the pain goes away only on the fifth or sixth day.”
When choosing where it is best to insert an implant, you need to understand that the question is not only whether the scar will be noticeable, but also the safety of the operation.
1. UNDER THE ARMMIT The operation is more difficult to do: from the incision site to the chest is 20 cm, and the surgeon’s fingers are no longer than 10 cm. You will have to work with instruments (and they are always worse than fingers), and the incision will be longer than under the chest.
2. AT THE EDGE OF THE AREOLA The scar in this place is completely invisible. But the operation is quite dangerous - the nipple risks losing sensitivity forever. And if the milk ducts are damaged, the patient will subsequently be unable to breastfeed.
3. UNDER THE BREAST In 90% of operated women, the incision is made here. This is the most convenient place for the surgeon, and therefore the safest. The length of the incision is 4.5 cm, and the scar in the fold is almost invisible.
4. NEAR THE BUTTON A technically complex operation, possible only with saline implants, which are inserted, rolled into a tube, then straightened and filled with liquid.
In Russia, the specialty “plastic surgeon” does not formally exist. That is, any surgeon who has graduated from an institute and attended a 144-hour mammoplasty course can perform breast surgery. As a result, the market is quite chaotic: in Moscow alone there are about 250 clinics. But there are only a few really good doctors. By what criteria can we calculate them?
* CERTIFICATE There is no point in even talking to a person without a medical education and a certificate of completion of courses in breast plastic surgery.
* EXPERIENCE Find out the history of the doctor: who he studied with, whether he completed an internship abroad, how long he has been practicing plastic surgery, how many operations he has performed. Be realistic: even a very experienced doctor is not able to perform more than two thousand operations in 15-20 years of practice.
* CLAIMS If the doctor assures that there have never been any complaints from his patients and problems are simply excluded for you, consider whether to refuse his services: not even the best surgeon can say this.
* IMPLANTS Make sure that you are receiving modern models of implants: Inamed, McGhan, Eurosilicone and Mentor - there are only four brands of high-quality implants on the entire Russian market. And they usually cost at least $800 for round ones and $1,300 for anatomical ones.
In December 2006, the 4th Congress of the Society of Plastic Surgeons of Russia was held. The question was brought up for discussion: is it correct to promise patients that implants will last a lifetime? The congress participants did not come to a consensus, but most leading surgeons are confident that this should not be done. Despite the fact that now every implant has a certificate of a lifetime guarantee, it is impossible to provide for everything. Modern implants are virtually eternal, but this is not the only factor. Every woman's breast changes every five years, so no surgeon can guarantee that replacement will not be required. In addition, a new generation of silicone implants has now appeared with a jelly filler instead of a liquid one. Which caused a wave of new operations to replace implants installed before 2000 with safer ones.
After discharge, you will need to undergo dressing changes every two days until the stitches are removed - usually on the tenth day. Swelling lasts for two to three weeks - due to hemorrhage in the mammary gland tissue. For two months you will have to constantly wear a supportive cotton bra with wide straps (to avoid stretch marks). And also limit your sex life, exclude sports, heavy lifting and hot baths. Also, do not smoke or take blood thinning medications such as aspirin. And sunbathing is prohibited for another whole year.
“The problem of possible complications is currently partly solved by a program for free replacement of mammary gland implants, which appeared in Russia. It allows you to change the old implant by paying only for the operation,” says Lyudmila Antonova, director of the Nike-Med aesthetic medicine clinic. Indications for replacement are the formation of a dense “capsule”, rupture of the implant, as well as dissatisfaction with the aesthetic result of the operation or a change in the patient’s complexion (for example, you have gained weight and want a larger breast size). Any surgeon who is a member of the Marketing Alliance for Aesthetic Mammoplasty can change the implant.
* RUPTURE A car accident, a fall, an injection - all this can cause the implant to rupture. Yet the main reason for ruptures is time and depreciation. According to the American Food and Drug Administration (FDA), up to 10% of implants fail after five years. Saline implants “deflate” and the saline leaks out, leading to inflammation. The thick filler of modern silicone implants does not leak.
* HEMATOMA During the day after surgery, blood usually accumulates under the skin. The body copes with small hematomas on its own. Larger ones do not dissolve and require opening to extract fluid.
* "CAPSULE" FORMATION The scars that form around the implant may be too dense. The breasts harden and lose shape, and even years after the operation, pain may occur. All this depends on the individual characteristics of healing, the woman’s immunity, etc.
* LOSS OF SENSITIVITY Approximately 10-18% of women do not feel their nipples at all, even five years after surgery.
* INFECTION, SUPUSION, BLEEDING Rare (2 in 1500 operations), but unpleasant complications that require removal of the implant with reintroduction of the same or smaller size after 4-6 months.
It is not recommended to repeat Pamela Anderson's feat. “The larger the breast size, the more serious the load on the spine, the more often a hard “capsule” is formed and the higher the chance of the body rejecting a foreign body. In addition, it is not easy to get rid of large implants: after their removal, the stretched tissues do not return to normal,” says Lyudmila Antonova, director.

CAN IMPLANTS CAUSE CANCER? This is not confirmed. On the contrary, the Americans conducted an experiment: they placed an implant in a rabbit next to a cancerous tumor. And - who would have thought! — on this side the tumor stopped growing altogether.
WILL I BE ABLE TO BREASTFEED MY BABY LATER? If the operation is successful and the mammary glands are not damaged, then yes, without any doubt. Moreover, the shape of the “artificial” breast in many patients did not change even after feeding the child.
