- I wouldn't say it's a whim. And this is now an accessible operation, that is, everything is quite possible. Girls who have had breast enlargement feel completely different. And now these are not some bomb results in terms of incredible sizes, but everything is neat and harmonious. Now patients come with real needs who understand that they have to live with it.

— What can girls with size zero expect?
— Zero size is an excellent source for endoprosthetics. We always look at the chest. It is impossible for a small girl with a size zero to get 500 “cubes” at once. More precisely, this is, of course, real, but it will not be very good - there will be problems. A year will pass, the tissues will stretch, and then - please.
— How effective is 3D visualization in consultations?
— There was a period when we actively used 3D, in fact it is a beautiful Photoshop, then we left this program. Probably now there is a need to return to it. But, to be honest, for mammoplasty in my practice this is of little relevance. It’s easier for me to show my work and specific examples of how it will be. 3D visualization is more relevant for noses (rhinoplasty). We also have trial implants. I also spend a lot of time during the consultation clarifying all the nuances in order to understand what exactly the patient wants.
— Let’s say the patient doesn’t want anyone to know that she has had breast augmentation. Is it possible to make it so that it is impossible to guess that the breast is “not your own”?
— If we are talking about avoiding any marks on the chest, then in this case we use endoscopy, that is, this is access through the armpit. Then there are no scars on the chest. And this is, of course, a good option. But it all depends on the source. In general, there are different types of scars. For some, the scar is perfect, for others it spreads a little. The scar matures for a year and eventually becomes lighter, then it can be polished with a laser.
— What other options for installing implants are there?
— They are standard: access through the areola; under the chest.
— Is the method of installing implants at the request of the patient or is it chosen by the surgeon?
- We are discussing this. But the last word, of course, belongs to the doctor. Or, let’s say, an enlargement and lift will be done. Then there is no point in placing an implant through the armpit and then leaving marks from the lift.
I often do mammoplasty on the areola. This is a very good way. There the optical zone is less noticeable.
To be honest, whether it is clear or not clear that an implant in the breast is usually not a priority. My priority is to fulfill the patient’s wishes for breast augmentation. Such moments when a patient after surgery says, “Oh, I wanted more,” are extremely rare for me.

— What harm does anesthesia cause to the body during surgery?
- Well, we examine patients. And this operation is small and not difficult. Lasts about a little over an hour. Sometimes it takes a little longer: I often make adjustments - I put in special sizers (sterile try-on implants) that can be adjusted in shape, size, placed and inspected. It is when the doctor or patient has doubts that we do this fitting. The operation itself takes place under general anesthesia. This is a modern safe anesthesia. And it's controlled.
— During the rehabilitation period, there are probably some unpleasant moments. What are they?
“I always say: in the first two or three days after the operation it will hurt a lot, so you need to take painkillers. Although in principle everything is tolerable. It’s not for nothing that we prescribe the patient on the first day, because this can already be done.
You also need to get used to compression garments, which you usually need to get used to for a month and a half. This is a special bra that is individually selected before surgery. Then, until three months, I recommend wearing sports-type underwear. Then, up to six months, wear a bra without wires to eliminate unnecessary pressure on the lower pole of the breast.
Also, you can’t lie on your chest while sleeping for three months. These are the moments you have to live through. After the end of the rehabilitation period there are absolutely no restrictions.

- All at once. I won’t even hide: yes, we see each other first through social networks, but then we still meet in a face-to-face consultation. It is also often a “sundress”: a relative or friend had surgery, she is happy with the result and recommends a doctor.
— The doctor and the patient’s idea of harmony does not always coincide. What can serve as a reason for refusing an operation?
- Yes, there were such cases. I recently had a situation when a girl came in who had her own rather large breasts. I ask her: “Show me what you want.” And she shows me options that are impossible for her to achieve. Or perhaps, but with scars that are unnecessary. And I told her that we will not get the result that she wants. After all, I communicate with patients after surgery. And it’s important to me that they are happy.
