How not to lose face

“If you think that I spend my days making small mammary glands into big ones and pumping out excess fat from bellies, you have an extremely misguided idea of ​​plastic surgery,” says surgeon Zubarev and, having secured my consent and “yes, my nerves are fine.” ", opens his photo album.

“This is how it is, plastic surgery is not all about aesthetics”: in a doctor’s professional gallery there is a horseman who, during a thunderstorm, accidentally “fitted” into a wire carrying a current of 4.5 thousand volts, a merry fellow who, after drinking too much, went to kiss his pit bull , a worker on whose head a log fell on his head during his shift, and many other folders. Behind each name is a personal tragedy of one scale or another and living - in the literal sense - evidence of surgical skill, photos before, after, after, after and a dozen more operations after.

“I know the face by heart, but it’s the most difficult thing to work with”: clicking the mouse, the surgeon briefly goes into the details of the operations, during which it is necessary, in essence, to re-create a person’s face. Bone by bone, nerve by nerve, muscle by muscle. This is a struggle not for beauty, but for the restoration of functions: a person can again open his mouth, eat, breathe, speak. The skin feels touch, heat and cold. Well, it doesn’t make passers-by faint with its mere appearance, which is also important. The processes are long and complex: the consequences of car accidents, accidents, work-related injuries and robberies in the “before” photo speak for themselves.

“Here, the electric current burned through the face to the bones, the lower jaw bone was completely exposed, the maxillary sinus was open - which means we need to make sure that after the operations the person lives without sinusitis, without fistulas, sinusitis, and breathes normally. The mucous membrane is formed, skin flaps are taken from the chest, and the bone material needed for reconstruction is taken from the ribs or fibula. It took a long time to straighten this face, for years. Performed 15 operations. Five years have passed from now to now, functions have been restored one hundred percent. The guy lives normally, his wife is smart and beautiful, she was nearby all the time. When loved ones help, people return to life more easily—I noticed this a long time ago.

Intern Zubarev performed his first operation in 1986. Which one exactly, he doesn’t remember and honestly admits it: “maybe a broken jaw... But I don’t remember, by God: in a week - thirty operations - purulent inflammations, fractures, planned operations, two a day.” That year, after graduating from OGMI. M.I. Kalinin, he began working in the department of maxillofacial surgery of City Clinical Hospital No. 11, where planned and emergency operations on the head and neck were performed. He worked there for twenty-five years, went into private medicine, operated in Moscow for several years, and returned to his hometown. In 2015, he received the Diamond Beauty international award in the field of beauty and health as the best plastic surgeon in Omsk - not just for “outstanding professional achievements,” but also for “regularly achieving successful postoperative results.”

But, as they say, nothing was foreshadowed - and I got into medicine by accident and unexpectedly for myself. I played volleyball and sambo, and after school I planned to go into professional sports, but my parents advised me to take a closer look at another profession. Moreover, of all the doctors in the family, only my aunt is a gynecologist. I entered medical school easily, choosing the dental department, and after the third year I became interested in surgery. He shrugs: “God probably sees who and where to direct... It led, it pulled. And yes, I'm in the right place. I would have lived another life exactly the same.”

If you ask Igor Alekseevich which of the greats he would like to meet and talk with, he will think for a minute and then say: “I would like to talk with my mother.” He is proud of his parents and grateful to his teachers: when he had a desire not only to restore faces, but also to work on improving them, he became seriously involved in aesthetic surgery of the face and body, studied with the luminaries of domestic and foreign surgery - with professor and academician Nikolai Milanov, with Wolfgang Hubisch from Germany, from the American Timothy Martin. But first, he still names those who participated in his formation here, on Omsk soil: Boris Davydovich Mironov, an oral and maxillofacial surgeon - “I learned everything from him.” Among his other teachers are professors of the medical academy Anatoly Mikhailovich Nikandrov, Anatoly Ksenofontovich Popov, Pyotr Ivanovich Ivasenko.

Why plastic surgery? "Because I like it. And because not everyone can do this,” Igor Alekseevich is sure that we need to mature before this. You cannot become a plastic surgeon right away; you must first master your specialty - maxillofacial surgery. “I can take my face apart down to the bones, then put it back together again. But in order to do this, you need to go through all the circles of surgical “paradise”: emergency room, trauma, inflammation... After any operation there can be complications, and a surgeon who has not bathed in either pus or blood will not know what to do after “guiding” beauty" - the so-called "pure" plastic surgery. To reach the top, to plastic surgery, you need to go through all these stages.”

“The point is that you take a practically healthy person - he is not sick - and begin to operate on him”: again and again Igor Alekseevich returns to the fact that plastic surgery consists of aesthetic and reconstructive components, which are necessary for medical reasons. Elimination of congenital or acquired defects, restoration of functions of chewing, breathing, vision, adaptation to social life - here a person cannot be denied. And when it comes to aesthetics, “sometimes the best plastic surgery is the one you didn’t do.”

Can a psychotherapist compete with a plastic surgeon? “In certain cases, yes, when there are no indications for surgery, and the requests do not coincide with reality,” says Zubarev and gives the example of the now textbook “nose like Angelina Jolie’s.” The most common thing he hears from female patients who have decided to transform themselves is: “Doctor, make me beautiful!” He admits that he still tolerates experiments with personal photos decorated in Photoshop, but when they show him photographs of other people, he orders them to immediately hide them and listen to him, the doctor, carefully: “I don’t take anyone who is stubborn during consultations.”

“To find your plastic surgeon, find out how many years he has worked, how many operations he has performed, what their long-term results are, talk to him, delve into his worldview”: Zubarev himself admits that, if he were in the client’s place, he would ask, among other things, the doctor about “Intelligence” asks how much this operation is necessary for him, whether it is indicated or not. “I can already understand from the answer that this doctor’s priority is money or my health.”

“At 50, I feel like I’m 35,” the surgeon hears. Age is not an indication for aesthetic plastic surgery, be it rhinoplasty, mastopexy or a circular facelift: self-awareness is important. “Not only the skin ages, but also the ligaments and muscles,” explains Zubarev. “And sometimes only a surgeon can bring one’s appearance into line with the years a person feels himself to be.” It won't work without hands. But there is no need to go against God. Sometimes he created one thing, a person asks for something else - anatomically impossible. You need common sense in everything."

His personal secrets of health, both external and internal, are: first of all, a good environment at home, “so as not to be nervous,” a mandatory Russian bathhouse, hot, with an oak broom and a swimming pool. If it’s meat, it’s fresh and from the market; if it’s vegetables, then it’s preferable from your own garden. And don’t smoke: “this is a common scourge. When you work in an emergency room, stress becomes the norm of life - how else can you relieve it during the working day?.. How did you quit later? Yes, it’s elementary, I just gave up and that’s it. I realized that enough is enough, enough is enough for me.”

And yet - that same harmony. To communicate with his son, daughter (she followed in her father’s footsteps - she chose medicine as a profession), with grandchildren - so far he has two of them. Go fishing - on a lake or on a mountain river. There, on the shore, think, breathe, be silent in silence. “Ordinary life goes on,” says the man who gives people back their faces and gives spiritual comfort. — Over the years, the desire to prove something to someone goes away. Priorities and values ​​change. Until the age of 50, you chase achievements, and then a realization comes: you cannot embrace the immensity, you must do well what you have learned and bring it to perfection. And pass on your experience to worthy and caring youth. But fighting against windmills is not at all necessary and sometimes even harmful - it is better to invest energy in saving someone’s life.”

Pyotr Nikolaevich Zubarev (1945—2020)

On December 22, 2022, after a serious long-term illness, at the 76th year of his life, Doctor of Medical Sciences, professor, retired colonel of the medical service, academician of the Military Medical Academy named after. Kirov, Honored Scientist of the Russian Federation, Honored Doctor of the Russian Federation and the Uzbek SSR, Professor of the Department of General Surgery at the Military Medical Academy named after. CM. Kirova Petr Nikolaevich Zubarev.

Pyotr Nikolaevich was born in Kazan in 1945. After graduating from school with a silver medal in 1963, he entered the faculty of training doctors for the missile and ground forces of the Military Medical Academy. CM. Kirov. His favorite subjects as a student were histology, operative surgery and topographic anatomy, and in the 3rd year his interest in surgery was finally determined. After graduating with honors from the Academy in 1969, Pyotr Nikolaevich was appointed to the position of resident officer of the surgical dressing platoon of the medical battalion in the city of Kyakhta in the Trans-Baikal Military District on the border with Mongolia.

In 1971, Pyotr Nikolaevich entered the postgraduate course at the Department of Advanced Training Surgery No. 2, headed by prof. M.I. Lytkin. The department intensively studied issues of emergency abdominal surgery, oncology, and portal hypertension surgery. Adjunct P.N. Zubarev skillfully combined active surgical activity with scientific creativity. In 1974 P.N. Zubarev successfully defended his thesis on the topic: “The main pathways of resorption of toxic substances in wound peritonitis” and was appointed to the position of teacher of the department.

The calm and measured life of the cathedral changed dramatically in January 1980, when Major of the Medical Service P.N. Zubarev without hesitation accepted the offer to lead the surgical service of the 40th Army as part of a limited contingent of Soviet troops in Afghanistan. In difficult conditions, at the very beginning of hostilities, Pyotr Nikolaevich organized an effective system for providing surgical care to the wounded in the warring army. Chief Surgeon of the 40th Army P.N. Zubarev constantly traveled to areas with the greatest sanitary losses, where he performed complex operations and trained young military surgeons. In May 1981, at the suggestion of Pyotr Nikolaevich, the First Scientific and Practical Conference of Surgeons of the 40th Army was held, which gave a powerful impetus to the scientific understanding of the combat experience gained.

In 1982, Pyotr Nikolaevich returned to Almamater. For participation in military operations on the territory of the Republic of Afghanistan P.N. Zubarev was awarded the Order of the Red Star in January 1982. In 1985 P.N. Zubarev was appointed to the position of deputy head of the department and clinic of advanced training surgery for doctors No. 2. A year later, Pyotr Nikolaevich defended his doctoral dissertation on the organization and provision of surgical care to the wounded in Afghanistan. Memories of the Afghan War were subsequently reflected in his book “Notes of an Army Surgeon” (1999, 2002). In 1988, Doctor of Medical Sciences P.N. Zubarev was awarded the academic title of professor.

Pyotr Nikolaevich made full use of his extensive experience as a surgeon-organizer during a business trip to the affected areas of the Armenian SSR in 1988, where a powerful destructive earthquake occurred.

In 1986 P.N. Zubarev headed the Department of General Surgery of the Military Medical Academy named after. CM. Kirov and remained its permanent leader for 25 years. The Department of General Surgery was founded in 1800 by prof. I.F. Bush and is the first department of surgical diseases in the history of domestic surgery. Working as a head, and subsequently as head of the department, allowed the famous surgeon and scientist to fully reveal his talent as a teacher and organizer. Pyotr Nikolaevich, possessing enormous capacity for work, infecting his students with energy, created a wonderful, close-knit team of like-minded people. Careful attitude towards teachers and their creative heritage, concern for a painless change of generations, a balanced combination of practical surgery with scientific research, the search for new ideas and their implementation in clinical practice have always been the fundamental principles in the work of Prof. P.N. Zubareva.

The range of scientific interests of Pyotr Nikolaevich is extremely wide. Throughout his long creative career, Prof. P.N. Zubarev was involved in the development of many current areas of surgery: surgery of the esophagus, gastric and duodenal ulcers, surgical oncology of the gastrointestinal tract, surgery of portal hypertension, wounds and wound infections, surgery of injuries.

Prof. P.N. Zubarev is the author of more than 300 scientific works, including more than 30 monographs, educational and methodological manuals. Under the direct supervision of Pyotr Nikolaevich and with his scientific consulting, 14 doctoral and 37 candidate dissertations were prepared and defended.

Petr Nikolaevich was a member of the Board of the All-Russian Society of Surgeons and Endovideosurgeons, a member of the editorial board of the journal “Bulletin of Surgery”, and Honorary Chairman of the Pirogov Surgical Society. For 20 years he was the permanent chairman of the surgical dissertation council of the Military Medical Academy.

Behind Pyotr Nikolaevich are hundreds of cured patients, numerous students and followers, a scientific school with a wide range of interests. His students have long since produced many worthy surgeons and scientists who continue the work of their Teacher and, occupying high positions in military and civilian healthcare throughout the country, remain faithful to his school.

Having devoted many years of hard and fruitful work, Pyotr Nikolaevich retained his charm, optimism and spiritual generosity and remained a man and professional of high moral principles, always ready to help his colleagues and patients.
