Types of eyelid plastic surgery

Depending on the volume and location of the intervention, there are several types of blepharoplasty operations. Each of them has its own characteristics and goals.

Upper eyelid blepharoplasty

Blepharoplasty is used in cases of pronounced sagging of the upper part of the eyelid, wrinkled skin folds above the eyes, narrowing of the field of vision, and pronounced subcutaneous fat hernias.

There are several modifications of this procedure:

• percutaneous blepharoplasty;
• “Asian eye” plastic surgery;
• transconjunctival blepharoplasty (without incisions).

Lower eyelid blepharoplasty

Lower eyelid surgery is performed if there are hernias or bags under the eyes. The purpose of such an intervention is to strengthen the musculofascial structures around the eye and remove or redistribute periorbital fat.

If it is necessary to change the appearance of the eyelid incisions and the shape of the eyes, modern cosmetology allows this to be done percutaneously or transconjunctivally. Removal of paint bags and lipofilling of tear troughs are also performed.

Circular eyelid surgery

This is the most extensive operation, during which plastic surgery is performed simultaneously on both the upper and lower eyelids using a small skin or conjunctival incision. The procedure takes up to 2 hours and allows you to comprehensively solve the aesthetic problems of the skin around the eyes.

Correction of the corners of the eyes

“Anatomy” of blepharoplasty of the lower eyelids (upper)

This type of operation involves two possible options for anesthesia: blepharoplasty under local anesthesia and plastic surgery of the periorbital region using intravenous anesthesia. The duration of the plastic surgery is about 50 minutes; if we are talking about circular blepharoplasty, it can take up to 2 hours.

Innovative technologies used during plastic surgery to correct the periorbital region make it possible to make the incision of the eyelids along the natural folds of the appearance barely noticeable, and to preserve the fatty tissue of the eyelids in the most unchanged volume. Moreover, modern medicine involves performing blepharoplasty without the need for external incisions and subsequent sutures. This technique is already actively practiced by leading clinics in Moscow and may soon take its rightful place among non-surgical methods of appearance correction. In general, blepharoplasty today involves applying a bandage to the eyes, which can be removed no earlier than 4-6 hours after completion of the operation.

Contraindications to blepharoplasty

Lower eyelid surgery is performed in two ways. The incision is made either along the ciliary edge of the lower eyelid, or on the inner side of the eyelid, through the conjunctiva.

Lower blepharoplasty with a subciliary incision is used when pronounced age-related changes in the eyelids and paraorbital area predominate.

Transconjunctival lower blepharoplasty is used primarily in young patients without excess skin and without pronounced wrinkles.

Preparing for anesthesia

Operations during which the eyelids are corrected are performed under general anesthesia (anesthesia), so the patient will need to consult an anesthesiologist. The consultation includes examination of the patient, identification of indications and contraindications, and determination of the surgical methodology. The patient is informed about the peculiarities of the course of anesthesia, possible reactions and complications.

8-12 hours before plastic surgery, you should stop eating and taking eye medications, and 4 hours before you should stop drinking. The anesthesiologist must be informed about medications that are constantly taken - he must either cancel them or adjust the dose.

Preparing for surgery

The scope of examinations before intervention includes:

• blood tests - general and biochemical;
• study of the blood coagulation system (coagulogram);
• electrocardiogram;
• laboratory testing for HIV, syphilis and hepatitis.

These tests can be taken in any laboratory, including ours. If there are concomitant diseases that may affect the course of the operation in the area under the eyes, consultation with a specialist is necessary. Thus, in the presence of cardiac pathology, consultation with a cardiologist is required, pulmonary pathology - with a pulmonologist, and with endocrine diseases - with an endocrinologist.

Performing eyelid surgery

Which technique to use is determined by the plastic surgeon individually for each patient. First of all, doctors try to use gentle techniques.

When operating on the upper eyelids, the incision is usually made along the natural crease, which makes the postoperative suture invisible. During the operation, excess skin is excised and hernias above the eyes are removed.

Lower eyelid surgery is performed in a similar way if there is excess skin or large fatty hernias under the eyes.

Transconjunctival eyelid correction without visible incisions on the skin is becoming increasingly popular. With this operation, access is carried out through the conjunctival surface; in this case, there are no postoperative scars. However, this method is only possible when there is no need to excise excess skin, but only requires the removal or redistribution of fatty tissue.

How the procedure is performed

Before the operation, an experienced anesthesiologist will come to your room, and after a conversation with him, your anxiety will leave you. Then everything is in a fog, usually in a pleasant fog.

Incisions are made in the natural folds of the lower and upper eyelids. This allows you to make the seam almost invisible. We distribute fat from the bags under the eyes, eliminate sagging, tighten tissues and fix them in a new position.

The final stage is the application of neat cosmetic stitches. After some time, the scars fade and become invisible.

Possible complications

After blepharoplasty, complications in the form of suppuration of wounds, corneal injuries, deterioration of vision, and asymmetry of the eyelids are extremely rare. Swelling, bruising and hematomas of the periorbital area after the intervention are a normal reaction of the body and disappear on their own after 2-3 weeks.

Possible side effects include excessive drooping of the corners of the eyes - this gives the face a sad, dissatisfied appearance. If there is insufficient tone of the lower eyelid or when too much skin is removed, ectropion occurs - inversion of the lower eyelid. In these cases, repeat corrective surgery may be required.

How is rehabilitation going?

You need to wear it for 7-10 days. During this period, it is recommended to sleep on your back.

Swelling and hematoma may increase, but you should not be afraid of this; these symptoms will soon disappear. 4 weeks after blepharoplasty, traces of surgical intervention completely disappear. You need to follow the doctor's recommendations for another 2 months. Don't forget the following rules:

• Take prescribed medications;
• Treat eyelids;
• Avoid physical activity;
• Avoid unnecessary touching of eyes;
• On applying makeup;
• Avoid alcohol and smoking;
• Don't strain your eyes.

After blepharoplasty, it is recommended to constantly use creams with an SPF factor. This will help your skin stay healthy and youthful longer. For 4 weeks you will have to give up visiting the bathhouse, sauna, swimming pool, and intense physical activity. Protect your eyes from possible injury. After a few months, the body will fully recover, the scars will heal - and it will be possible to evaluate the final result of the operation.

Postoperative period after blepharoplasty

Immediately after the procedure, the operated area is covered with a sterile bandage, and the skin sutures are removed after 4-5 days. To reduce swelling of the upper and lower eyelids, it is recommended:

• limit head movements;
• lean forward as little as possible;
• stop wearing contact lenses (up to 2 weeks);
• do not engage in sports or heavy physical work (up to 1 month);
• carry out hygienic treatment of eyes and skin around.

Wearing sunglasses and eliminating any irritants in front of the eyes (reading, working at the computer) is recommended for 1.5-2 weeks after eyelid blepharoplasty. After this, the patient can return to their normal lifestyle; it is allowed to use hypoallergenic cosmetics in small quantities. The final transformation of the eyes and face can only be assessed after 6 months.
