Postoperative period:

Plastic surgery on the mammary glands imposes some restrictions in the postoperative period. After the operation, the woman will need to wear special compression underwear (bra) for 15-30 days, which it is advisable to purchase in advance. It is not recommended to use bras with a wire frame. RECOVERY AFTER MAMMOPlasty:

Breast reduction

Breast reduction is performed in women with macromastia (large size), and in men with gynecomastia. The operation is indicated in cases where large breasts cause physical or mental discomfort.

There are several known surgical techniques for breast reduction surgery. Today, preference is given to techniques that involve small incisions and short scars. One of these gentle techniques is the SPAIR technique by American surgeon Dennis Hammond. When using it, there is no need to make large incisions; after the operation, small scars remain around the areola and on the lower part of the breast.

The operation is performed under general anesthesia. It lasts 2–3.5 hours and ends with the application of absorbable sutures. The final result will be visible in about six months.

After performing reduction mammoplasty, a 24-hour hospital stay is required. The patient must limit physical activity for two months.

If the left and right mammary glands are different in size, this condition is called breast asymmetry. It can be eliminated in different ways: by reducing or enlarging one of the mammary glands or using implants of unequal sizes. The duration of the operation and its other features depend on the operating technique used.

Breast lift

Mastopexy is a plastic surgery performed for ptosis of the mammary glands. A sign of ptosis is the descent of the nipple below the inframammary fold. breast lift options, the surgeon chooses the optimal one, depending on the individual characteristics of the patient and her wishes. With the first degree of ptosis, this can be, for example, periareolar mastopexy, with the second degree of ptosis - vertical mastopexy. You can lead an active lifestyle already on the 4th–5th day after surgery. You will need to wear compression garments for 3–4 weeks, and physical activity will need to be limited for a period of 1.5 to 2 months. The aesthetic effect will fully appear approximately six months after mastopexy.

Nipple correction


Correction of areolas and nipples is carried out under local anesthesia. The goal of areola correction is to reduce them to normal size, give them a rounded shape and make them symmetrical. Indications for nipple correction are their retraction or too large size.

All of the above types of operations belong to aesthetic mammoplasty, they are performed in order to improve the appearance of the mammary glands.

Restoration and reconstruction of mammary glands

Reconstructive mammoplasty solves a different problem - restoration of the mammary gland in its absence. The need for such an operation often arises after removal of a malignant tumor.

Breast endoprosthetics - breast augmentation surgery using implants filled with silicone gel - is a reliable and safe method of correcting the size and shape of the mammary glands, as well as giving them elasticity.

Reasons for undergoing plastic surgery for breast augmentation may include dissatisfaction with the size of the mammary glands, loss of shape after childbirth and breastfeeding, visible breast asymmetry, structural features of the breast (tubular form), as well as the desire to restore the breast after mastectomy surgery (removal of the mammary gland for breast cancer). glands).

The history of breast augmentation began in 1889, when the first attempt was made to enlarge using paraffin injection. In 1895, surgeon Vincenz Czerny performed the first transplant of autologous fat, taken from a lipoma in the thigh, to a woman who had had her breasts removed. Then began a period when rubber and paraffin, ox cartilage and gutta-percha, ivory and glass implants, synthetic materials - foam, polyurethane, polyethylene and polyvinyl sponge - were used to enlarge the breasts, but the use of these fillers only led to complications and the need to remove these materials.

In the 40-60s of the last century, liquid silicone was used for breast enlargement, which was injected. This technique was subsequently banned, since the introduction of liquid silicone led to a large number of problems - the formation of fibrosis, infection, tissue necrosis, fistula formation, and gel migration. But perhaps the use of this particular substance prompted Houston plastic surgeons Thomas Cronin and Frank Gerow to invent the first breast enlargement implant in 1961, almost in the form in which we see it and now.

The history of the development of this method has gone through many tragic and curious moments, but the basic concept has not changed. The material must be encased!

The shell of the implants, which was initially single-layer and smooth, also improved, and in the 1980s implants with a textured multi-layer shell appeared. The textured shell does not allow the formation of a rough capsule, which is the prevention of capsular contracture, and the presence of several layers does not allow the gel to leak through - “sweat” through this shell.

Currently, 5th generation implants are used, which are absolutely safe.

The filler for implants has also changed somewhat. Nowadays, implants typically use a “cohesive gel” that has the consistency of jelly (standard) or marmalade (highly cohesive).

The choice of the type of implants is made during a consultation after an examination and taking the necessary measurements. It is important to remember that correctly selected implants will give you the opportunity to get beautiful breasts.

The shape of implants can be anatomical (drop-shaped) or round. Although I install anatomically shaped implants more often, I can say that round-shaped implants also allow you to get a beautiful breast shape - it all depends on the wishes and individual anatomical characteristics of the patient. During the consultation, compression garments are selected and a surgery date is set.

There are three access options:

  • Submammary (in the fold under the breast)
  • Periareolar (along the lower edge of the areola)
  • Transaxillary (axillary)

Implants can be installed directly under the gland, under the fascia of the pectoralis major muscle, under the muscle and under the gland (two-plane installation) and completely under the muscle.

The last option is used extremely rarely.

Regardless of the chosen access and installation method, the mammary gland remains intact, and neither sensitivity nor the ability to lactation is affected.

The choice of installation plane does not affect the ability to play sports.

Before the operation, it is necessary to undergo a preoperative examination . Typically, laboratory tests are carried out 5-7 days before surgery directly in the clinic or at your place of residence. The list of mandatory studies includes an ultrasound of the mammary glands and an examination by a mammologist.

On the day of surgery, the choice of technique and volume of implants is discussed again, photographs are taken, and preoperative markings are performed.

The operation “Breast endoprosthetics” is performed only under general anesthesia. The duration of the operation is 1-2 hours.

I prefer to perform access along the lower edge of the areola - the seam is very cosmetic, and over time, the traces of the operation become completely invisible. Implants are placed under the gland or under the muscle; the second option (two-plane installation of implants) is more often practiced. In this case, more than half of the implant in the upper section is covered by the massive pectoralis major muscle, which allows for a smooth transition from the chest to the gland itself, and, accordingly, to avoid the formation of a “step” above the chest in the future. During the operation, the mammary gland is not damaged, so the operation itself does not lead to disruption of breast sensitivity and the ability to lactation. During the operation, a cavity is formed into which the implant is then placed. Sutures are placed on the internal tissues and absorbable sutures on the skin.

After the operation, compression garments are put on and must be worn for 3-4 weeks. Hospitalization after such surgery is 1 day. The next day after the dressing you can go home. After discharge, you need to come back for dressings several times. During the first 3-5 days, you may experience pain. There is no need to remove sutures after breast replacement; the bandages allow you to take a shower after 5-7 days. To avoid complications such as bleeding, implant displacement, capsular contracture, etc., you must strictly follow the doctor’s recommendations: for the first one and a half to two weeks, avoid any physical activity, including driving. Over time, there will be fewer and fewer restrictions, and within a couple of months you will be able to return to your normal lifestyle.

You will see beautiful breasts in the mirror 2-3 weeks after surgery, but the final result of breast replacement can be assessed only after 6 months.
