Dmitry, tell us how you became a plastic surgeon?

At the very beginning of my career, I was a general surgeon. Later he retrained and began to gain more practice in thoracic surgery - he performed lung resection, removed tuberculomas, and treated patients who had complications from tuberculosis. I started my career in a regional hospital, which was located at a correctional colony, that is, our patients were prisoners. They often had various injuries - ruptures of the ears or soft tissues of the nose, fractures of the nose. At their request and desire, I slowly began to enter the field of reconstructive aesthetic surgery. However, I lacked experience and knowledge, so I submitted documents to RUDN University, where I received retraining. Since 2014, I have been exclusively engaged in aesthetic surgery. Although my experience as a surgeon is already 11 years.

At the moment I perform operations on a fairly wide range. For the face: blephoroplasty, rhinoplasty, otoplasty, facelift, that is, facelift. On the body: combined operations, recovery after childbirth: chest, abdomen, body liposculpture using the Vaser device. Vaser is an ultrasonic liposuction device, a new product that has proven itself very well in body liposculpture. I also perform surgeries to sculpt the cheek area, called “French cheeks.” I remove Bisha's lumps and suture the buccal muscle. As a result, we get a stable frame that does not float after removing lumps, and the screed looks even, so the effect of sophistication in the middle third of the face is more correct, pronounced and natural.

What is conservation rhinoplasty?

Conservation rhinoplasty is the concept of working out the structures of the nose with minimal trauma and maximum results.
We do not destroy the surface layers and do not remove either the bone or cartilage structure of the nasal dorsum. But at the same time, we are working on the internal structure in order to lower the back without destroying it. Also, by certain manipulation of the cartilage, we create the effect of stability of the tip of the nose so that it does not droop in the future. At the moment, I am trying to get as close as possible to theorinoplasty - this is another branch of conservation rhinoplasty, the author of which is the famous surgeon from Istanbul Teoman Dogan. An ideally performed theorinoplasty does not require the application of splints, plaster, tampons, or any splints. Patients can be sent home the same evening with minimal swelling and bruising.

How does the preparation process for rhinoplasty work?

The preparation is standard: the patient undergoes a CT scan of the nose, septum and paranasal sinuses. We are looking at whether there may be problems with the septum in the future, how much it can block the nasal passages, and whether there is an ENT pathology. After this, we proceed to further examination. Everything is the same as for a regular elective operation. The only thing we ask patients to avoid taking hormones, vitamins, blood thinners for a week is to avoid bruising, hematomas and, most importantly, to avoid massive profuse bleeding.

How long does recovery take? How quickly does the patient return to his normal rhythm of life?

Recovery with preserving low-traumatic techniques is quite rapid. After a week, we remove the splint, if it was needed at all. By this time, there are no longer any primary bruises or swelling left. Next comes simply adaptation of the nose. Of course, it will take several months to form; we get an approximate stable picture after six months, so patients need to be patient. Within a week, patients can easily return to their normal rhythm of life, and after a month they can begin physical activity. The only thing is that athletes who spar need to be careful - they should not injure their nose for a year so that re-correction is not required.

Who would you not recommend to get rhinoplasty? What are the contraindications?


Contraindications for rhinoplasty are standard - these are exacerbations of chronic diseases, acute inflammatory processes, colds, viral diseases. It is not advisable to perform surgery during menstruation. If there are any physical health problems, we send the patient to a therapist, and only after his approval can we take him for surgery. As for autoimmune diseases, we also look at how much everything is compensated. For example, diabetes. If the blood sugar is very high, and the patient does not correct the indicators, does not work with an endocrinologist, we will not take him either. With multiple sclerosis of the cerebral vessels, we also look at what kind of compensation the body has, how complicated everything is, what attacks there were during the year. This also applies to epilepsy and prolonged depression. Usually patients try to keep silent about this, but we start by asking whether they are taking any additional medications and why they are doing this. Many admit that they have long-term insomnia, depression, etc. We refer such patients for consultations with a psychiatrist, where he already answers whether this patient can even undergo such an operation and how he will tolerate it, what the rehabilitation process will be like.

I would not recommend rhinoplasty to people who do not understand the essence of this operation. This is not some kind of magic pill that you take and wake up in the morning with a new nose. If patients do not understand that this is a surgery with its own risks, then such people should not have surgery. I also refuse patients who have come up with an overly ideal nose in their head and are sure that they will get exactly the same one. I explain that we strive for our ideal pictures, but we have swelling, skin density, a certain structure and anatomy of the nose, so we are 70% closer to any modeling. This doesn't mean it will be worse.

It often happens that we do one modeling, but in fact the nose turns out much more beautiful and natural. Here there must be full awareness of the step that patients are taking, and an understanding that they themselves are responsible for this step. We, as surgeons, must listen to them completely, explain everything, show them and determine whether the patient is mentally ready for this or not.

In general, how far has plastic surgery come?

At the moment, plastic surgery is moving forward with five-mile strides.

For example, ultrasound was not used as often as it is now. If we take liposculpture of the body, it is easier and better to perform it with ultrasound: not just collect pieces of fat from the subcutaneous fatty tissue, but turn it into an emulsion, and then create a skin tightening effect with a sound wave.

Regarding rhinoplasty, many rough, heavy and chopping instruments have also been replaced by ultrasound. I actively use Piezotome - this is a device that allows thin nozzles using ultrasound to make a stable, controlled thin section of the bone structure without injuring soft tissues. Here, rehabilitation occurs in the shortest possible time, and the course of the operation itself is predictable. We can also use ultrasound to break up fat traps on the lower back and sides of the abdomen, which do not go away either from diets or from exercise, because this is such a special fat, overgrown with fibrous layers. As a result, the result can be preserved for decades.

Our specialists create their own techniques and methods for using the same ultrasound. I am glad that there are a lot of young talented surgeons. The older generation of our colleagues shares their enormous experience, the methods that they have developed over 15-20 years of practice, we eagerly grab them and actively use them. We are already at a good level in aesthetic surgery, and we will be even higher.
